The paraurethral cyst or Skene's duct cyst is a rare congenital anomaly in the female newborn. It appears like a round, yellow or orange-colored cystic mass on either side of the urethral meatus. The reported incidence is 1 in 2.000 to 7.000 female births. The low frequency of the Skene's duct cyst and the little awareness of it and its benignity may lead to wrong diagnosis that underestimates its incidence. There are authors that agree with surgical treatment of paraurethral cyst to obtain an early resolution, even though a majority agrees to manage conservatively.
Disponible en Internet el 23 de mayo de 2015La incontinencia urinaria en la edad pediátrica es una causa muy frecuente de consulta, más prevalente en niñas, y que varía entre el 3 y el 12% en niños con edades comprendidas entre los 6 y los 8 años, y entre el 1 y el 12% en edades comprendidas entre los 10 y los 12 años 1 .El manejo diagnóstico de la incontinencia urinaria diurna en niñas sin alteraciones neurológicas, se basa fundamentalmente en la historia clínica y la exploración física, el diario miccional y los estudios urodinámicos no invasivos. Solo en ocasiones, cuando la incontinencia se asocia a otros sínto-mas de disfunción del tracto urinario inferior, es necesaria la realización de otras pruebas complementarias, destinadas a la caracterización de la anatomía y/o dinámica vesical como la urodinámica/videourodinámica e incluso, en ocasiones, la cistouretrografía 2 .La incontinencia urinaria secundaria al reflujo vesicovaginal o uretrovaginal está considerada una causa marginal de incontinencia urinaria, afecta a niñas que tienen pérdidas de orina en los minutos posteriores a la micción y no se asocia con otros síntomas. Sin embargo, el paso de orina durante la micción a la vagina es un fenómeno conocido tanto en niñas como en adultas, y que no siempre es causa de síntomas en forma de incontinencia urinaria 3 .El reflujo vaginal sintomático, o causante de incontinencia urinaria, se define como la incontinencia que aparece en niñas con control de esfínteres, en las cuales la fuga de orina aparece en los 10 min siguientes a la micción espontá-nea, siendo la micción normal y no encontrándose anomalías anatómicas en la exploración física 4 .Correo electrónico: Rosa.romero@ouh.nhs.uk Como muestra la revisión de los 3 casos descrita por Fernández Ibieta et al. 5 en el presente número de la revista, siendo el reflujo vaginal una causa reconocida de incontinencia urinaria en niñas en edad prepuberal, en ocasiones pasa desapercibida. Como en los casos descritos por los autores, no es raro que se llegue al diagnóstico tras una serie de investigaciones 5 o tras historia de fracaso terapéutico del manejo de la incontinencia urinaria. De hecho, la incidencia real del reflujo vaginal como causa de incontinencia urinaria es desconocida 1 .En la serie presentada por Bernasconi et al. 1 , en la que se estudian 39 niñas con incontinencia urinaria diurna, la causa más frecuente de incontinencia diurna fue el reflujo vaginal. Las pacientes con reflujo vaginal presentaban, además, una mayor edad media al diagnóstico, un mayor índice de masa corporal y, en 2 casos, sinequia vulvar. Siendo la obesidad un factor de riesgo estadísticamente significativo en este grupo de niñas, hecho que ha sido documentado por otros autores 3 .Aunque la ausencia de anomalías anatómicas es uno de los criterios diagnósticos del reflujo vaginal, en mi propia experiencia y en la de otros autores, hay niñas que tienen anomalías menores, como uretras hipospádicas, sinequias vulvares o labios menores redundantes que hacen más fácil que la orina quede ac...
en el caso del epidídimo izquierdo muy necesaria para evitar el tratamiento de sustitució n hormonal cró nico. Ninguno de los casos que hemos revisado han presentado evidencia de recidiva o diseminació n, por lo que el tratamiento conservador no compromete el pronó stico de la enfermedad. Nuestro paciente está libre de enfermedad 2 añ os despué s de la cirugía, tal como lo demuestra el control ecográ fico. No creemos que sea necesario un seguimiento a largo plazo para los casos unilaterales. En casos bilaterales, una valoració n de la funció n hormonal parece apropiada. Los leiomiomas de epidídimo son tumores poco frecuentes pero pueden presentarse en la prá ctica clínica diaria. El diagnó stico se basa en la palpació n y el estudio ecográ fico. Debido al excelente pronó stico, la exé resis podría obviarse, pero la ecografía no suele ser suficientemente fiable para el diagnó stico definitivo, por lo que la exé resis quirú rgica completa es el tratamiento má s recomendable cuando nos enfrentamos a una masa só lida paratesticular. Una vez que conocemos que se trata de un leiomioma hay que evitar los tratamientos agresivos dado que, como en nuestro caso, puede aparecer otro tumor en el epidídimo contralateral, incluso añ os despué s.
