Цель: оценить роль интраоперационной лучевой терапии (ИОЛТ), в сравнении с обычной дистанционной лучевой терапией (ДЛТ), при комбинированном лечении рака молочной железы (РМЖ). Материал и методы: Исследование было проведено на базе отделения опухолей молочной железы ГБУЗ «Клинический онкологический диспансер №1» Министерства здравоохранения Краснодарского края. Основную группу исследования составили 104 пациентки, которым было выполнено органосохраняющее хирургическое лечение (ОСО) в комбинации с ИОЛТ, группу сравнения - 180 больных, после ОСО в комбинации со стандартной ДЛТ. Анализ косметических результатов лечения проводили с помощью 4х-бальной Гарвардской шкалы, для оценки поздней токсичности использовалась модифицированная для рака молочной железы шкала LENT-SOMA. Статистический анализ был проведен при помощи пакета STATISTICA (v.10. Tibco. USA). Результаты: Показатель безрецидивной выживаемости у женщин основной группы на протяжении 36-48 (медиана 43±0,8мес) месяцев наблюдения и группы контроля на протяжении 36-48 (медиана 45±0,6мес) месяцев наблюдения составил 95,0% и 97,0%, соответственно (р>0,05). Локальные рецидивы были диагностированы практически с одинаковой частотой в группах сравнения: 3,8% - в группе ИОЛТ и 3,3% - в группе ДЛТ. Косметические результаты лечения в группах достоверно не отличались, однако, отмечается значительное снижение частоты возникновения гиперпигментации после ИОЛТ, в сравнении со стандартной ДЛТ. Выводы: Применение ИОЛТ при ОСО у пациенток РМЖ позволяет существенно уменьшить лучевую нагрузку, продолжительность и стоимость лечения, что особенно важно для пациентов старшего возраста, работающих женщин и тех, кто живет на значительном удалении от места проводимого лучевого лечения. Частота локальных рецидивов в сравниваемых группах достоверно не отличается. Предложенный комбинированный метод лечения РМЖ способствует снижению частоты поздних лучевых осложнений, таких как гиперпигментация и фиброз, что способствует улучшению косметических результатов. Aim of the study. a comparative assessment of perioperative parameters and short-term outcomes of robotic and laparoscopic surgery for colorectal cancer (CRC). Material and methods. The results of surgical treatment of 101 patients with colorectal cancer underwent robotic (I group, n = 30) and laparoscopic (II group, n = 71) radical operations were analyzed. Results. Surgery duration in I group ranged from 140 to 420 minutes (on average 260,9 ± 80,1 min.), in II group - 120-465 minutes (on average - 243,3 ± 72,9 min.; P = 0,314). Blood loss in I group averaged on 100 ± 29,4 ml (50,0-150,0 ml) compared to 141,7 ± 76 ml in II group (the difference is statistically significant, P = 0,006). Conversion rate, complications, duration of the postoperative period did not have a statistically significant difference between the groups. The study of circular resection margins after robotic surgeries confirmed the radical intervention (R0-resection) in all cases. Differences in the number of lymph nodes (12-22) between the groups were not observed. Conclusion. Safety and efficacy of robotic colorectal surgery is comparable to laparoscopic surgery. Long-term results require further study.
Objective: to compare the immediate and long-term outcomes of patients with early breast cancer treated with intraoperative radial therapy depending on the biological subtype of breast cancer.Materials and methods. We prospectively evaluated number of recurrences, cosmetic effect and early treatment results of 104 patients with early breast cancer aged 66.72 ± 0.68 years old. The mean follow-up period was 36 months. The mean dose on the surface of applicator was 17.8 Gy, on the depth 0.5 cm – 8.8 Gy, on the depth 1.0 cm – 5 Gy. The mean time of radiation was 22.15 min ± 28.09 sec.Results. The local recurrence was in 3 patients. The first patient had triple negative breast cancer subtype, the second patient had luminal B HER2+, and the third one had luminal B HER2– subtype. Relapses occurred in 7, 14 and 20 months after the end of treatment respectively. The recurrence rate in patients with luminal B biological subtype was 10.71 %; in patients with triple negative subtype was 20 %. All recurrences were found in the area of the postoperative scar.Conclusion. The obtained results question the rationale for the use of the demonstrated method in patients with luminal B and triple negative molecular subtypes of tumors.
Colorectal cancer ranks third after breast cancer in terms of incidence and second after lung cancer in terms of mortality.Management of rectal cancer requires a multidisciplinary approach, with the surgical management playing the main role. There are currently three resective techniques that complement the traditional open surgery: laparoscopic surgery, robotic surgery, and transanal total mesorectal excision.Rectal cancer in elderly patients is particularly hard to diagnose and treat surgically due to multiple comorbidities and limited functional reserve. Treatment of such patients may be associated with poorer outcomes after both open and minimally invasive surgery.This article reviews the current state of advances in minimally invasive surgery for rectal cancer in general and in elderly patients in particular.
