Background: Lung cancer surgery is associated with a high incidence of postoperative pulmonary complications (PPCs). Preliminary evidence suggests that ERAS processes can reduce overall incidence of PPCs as short-and longterm recovery improved by supporting units to adopt evidence-based care. However, the evidence is inconclusive due to insufficient high-level studies in this research field. No well-designed, adequately powered, randomized controlled trials (RCTs) have investigated the effects of pulmonary rehabilitation based ERAS program (PREP) on post-operative pulmonary complications, pulmonary function, and health related quality of life following lung cancer surgery. Methods:The PREP trial is a pragmatic, investigator-initiated, multi-center, randomized controlled, parallel group, clinical trial. Five hundred patients scheduled for minimally invasive pulmonary resection at six hospitals in China will be randomized with concealed allocation to receive either i) a pre-operative assessment and an information booklet or ii) a pre-operative assessment, an information booklet, plus an additional education, a 30-min pulmonary rehabilitation training session and the post-operative pulmonary rehabilitation program. The primary outcome is incidence of PPCs defined with the Melbourne Group Scale diagnostic scoring tool. Secondary outcomes include incidence of cardiopulmonary and other complications, pulmonary function, cardiopulmonary endurance, muscle strength, activity level, health-related quality of life (HRQoL), pre-and post-operative hospital length of stay (LOS), and total hospital LOS. Discussion: The PREP trial is designed to verify the hypothesis that pulmonary rehabilitation based ERAS program reduces incidence of PPCs and improves pulmonary function and HRQoL in patients following lung cancer surgery. This trial will furthermore contribute significantly to the limited knowledge about the pulmonary rehabilitation based ERAS program following lung cancer surgery, and may thereby form the basis of future recommendations in the surgical community.
Background:Long non-coding RNAs (lncRNAs) are significant contributors to various human malignancies. The aberrant expression of lncRNA LINC00894 has been referred to in various human malignancies. Consequently, our aim is to illustrate the role of LINC00894 and its underlying mechanism in the development of papillary thyroid carcinoma (PTC). Method:Bioinformatics analysis of differentially expressed RNA from TCGA and GEO datasets and selected the target lncRNA LINC00894.The analysis of SRAMP found that there are abundant M6A methylation sites in LINC00894. And further analysis from StarBase, GEPIA, and TCGA datasets to find related differentially expressed genes METTL3. The Colony formation and CCK8 assay confirmed the relationship between LINC00894, METTL3, and the proliferative capacity of PTC cells. The analysis of AnnoLnc2, Starbase datasets, and meRIP-PCR, qRT-PCR confirmed the influence of Mettl3-mediated modification of M6A on LINC00894. The study employed KEGG enrichment analysis as well as Western blotting to investigate the impact of LINC00894 on the expression of proteins related to the Hippo signaling pathway. Results:LINC00894 hypoexpression was detected in PTC tissues and cells and even lower in PTC with lymphatic metastasis. LINC00894 inhibits the lymphangiogenesis of vascular endothelial cells and the proliferation of cancer cells. METTL3 enhances PTC progression by upregulating LINC00894, which relies on enhancing the LINC00894 mRNA stability through the M6A-YTHDC2-dependent pathway.LINC00894 may inhibit PTC malignant phenotypes through the Hippo signaling pathway. Conclusion: The METTL3-YTHDC2 axis stabilizes LINC00894 mRNA in an M6A-dependent manner and subsequently inhibits tumor malignancy through the Hippo signaling pathway.
With China's strong support for innovation and development, the policies of government guidance funds are an important driving force for leading innovation and economic transformation, and the effect of its related policies is of great significance. This paper selects 518 policies related to government guidance funds in the Beijing–Tianjin–Hebei region from 2005 to 2018 as the research object and constructs a policy efficiency evaluation indicator system from three dimensions of policy intensity, policy objectives and measures. The analytic hierarchy process (AHP) and entropy method are used to weight the output level of policy innovation performance and economic performance of government guidance funds, and grey relational analysis (GRA) is used to measure the relationship between policies of government guidance funds and innovation performance and economic performance, and corresponding policy recommendations based on the research results. The results of the research show that the policy efficiency of the government guidance funds in Tianjin and Hebei is relatively high, and the policy efficiency of the Beijing government guidance funds needs to be improved. In general, the policy efficiency of the government guidance funds needs to be further improved.
