The use of an isotope label-free absolute quantification method for the simultaneous determination of 9 CYPs and 5 UGTs in human liver microsomes reveals that expression levels of CYPs and UGTs in human liver are severely impact by HCC, which could impact drug metabolism, disposition and pharmacotherapy.
Minimal hepatic encephalopathy (MHE) is associated with changes in functional connectivity. To investigate the patterns of modular changes of the functional connectivity in the progression of MHE, resting-state functional magnetic resonance imaging was acquired in 24 MHE patients, 31 cirrhotic patients without minimal hepatic encephalopathy (non-HE), and 38 healthy controls. Newman's metric, the modularity Q value, was maximized and compared in three groups. Topological roles with the progression of MHE were illustrated by intra- and intermodular connectivity changes. Results showed that the Q value of MHE patients was significantly lower than that of controls (P < 0.01) rather than that of non-HE patients (P > 0.05), which was correlated with neuropsychological test scores rather than the ammonia level and Child-Pugh score. Less intrasubcortical connections and more isolated subcortical modules were found with the progression of MHE. The non-HE patients had the same numbers of connect nodes as controls and had more hubs compared with MHE patients and healthy controls. Our findings supported that both intra- and intermodular connectivity, especially those related to subcortical regions, were continuously impaired in cirrhotic patients. The adjustments of hubs and connector nodes in non-HE patients could be a compensation for the decreased modularity in their functional connectivity networks.
With the development of telemedicine and e-health, usage of online health communities has grown, with such communities now representing convenient sources of information for patients who have geographical and temporal constraints regarding visiting physical health-care institutions. Many previous studies have examined patient–provider communication and health-care service delivery in online health communities; however, there is a dearth of research exploring the relationship between patients' level of self-disclosure and the establishment of patients' trust in physicians. Consequently, this study aims to explore how patients' self-disclosure affects the establishment of patients' trust in physicians. “Good Doctor,” which is a China-based online health community, was used as a data source, and a computer program was developed to download data for patient–physician communication on this community. Then, data for communications between 1,537 physicians and 63,141 patients were obtained. Ultimately, an empirical model was built to test our hypotheses. The results showed that patients' self-disclosure positively influences their establishment of trust in physicians. Further, physicians' provision of social support to patients showed a complete mediating effect on the relationship between patients' self-disclosure and patients' establishment of trust in physicians. Finally, evidence of “hope-for-help” motivation in patients' messages weakened the effect of patients' self-disclosure when physicians' social support was text-based, but strengthened it when physicians' social support was voice-based.
Based on cloud computing theory and service-oriented architecture (SOA) design pattern, a smart education management platform is designed by using cloud computing and artificial intelligence technology. The platform is deployed in the server cluster environment, with Hadpoop managed storage cluster as the data storage center. For the education management, education portal and remote classroom, the corresponding services are launched, which has the advantages of intelligent and efficient, massive data access and intelligent collaborative management. The test results show that the platform can be used normally in the ports of computer, mobile phone and tablet computer, and can successfully complete the basic operations such as user registration and login, educational administration, and content storage. The results of database stress test show that the total time consumption of sequential read, sequential write, random read and random write is 752s, 312s, 968s and 211s, respectively when millions of simulated data are inserted into the database, which indicates that the database can support large-scale data access. The stress test results of content storage service show that when the number of clients is adjusted to 500, 1000 and 3000, the output quantity of each read-write interface can be maintained at about 3500 pages/min in 60s, indicating that the system can still run stably under the condition of high concurrency. The management platform discussed has practical significance to promote the development of intelligent and information-based education management.
<b><i>Introduction:</i></b> Long noncoding RNAs (lncRNAs) have been reported to be involved in the occurrence and development of various diseases. This study was to investigate the role of lncRNA-H19 in the transition from acute kidney injury (AKI) to chronic kidney disease (CKD) and its underlying mechanism. <b><i>Methods:</i></b> Bilateral renal pedicle ischemia-reperfusion injury (IRI) was used to establish the IRI-AKI model in C57BL/6 mice. The expression levels of lncRNA-H19, miR-196a-5p, α-SMA, collagen I, Wnt1, and β-catenin in mouse kidney tissues and fibroblasts were determined by quantitative real-time PCR and Western blotting. The degree of renal fibrosis was evaluated by hematoxylin and eosin staining. The interaction between lncRNA-H19 and miR-196a-5p was verified by bioinformatics analysis and luciferase reporter assay. Immunohistochemistry and immunofluorescence were used to evaluate the expression of α-SMA and collagen I in kidney tissues and fibroblasts of mice. <b><i>Results:</i></b> lncRNA-H19 is upregulated, and miR-196a-5p is downregulated in kidney tissues of IRI mice. Moreover, miR-196a-5p is a direct target of lncRNA-H19. lncRNA-H19 overexpression promotes kidney fibrosis and activates fibroblasts during AKI-CKD development, while miR-196a-5p overexpression reversed these effects in vitro. Furthermore, lncRNA-H19 overexpression significantly upregulates Wnt1 and β-catenin expression in kidney tissues and fibroblasts of IRI mice, while miR-196a-5p overexpression downregulates Wnt1 and β-catenin expression in kidney tissues and fibroblasts of IRI mice. <b><i>Conclusion:</i></b> lncRNA-H19 induces kidney fibrosis during AKI-CKD by regulating the miR-196a-5p/Wnt/β-catenin signaling pathway.
