Granulocytic sarcoma, also known as myeloid sarcoma or chloroma, is a neoplasm of the myeloid cells that can arise before, concurrent with or following haematolymphoid malignancies, mostly associated with Acute Myeloid Leukaemia (AML). It is an uncommon extramedullary manifestation of AML. We report the case of a 13-year-old girl who presented with a painless lump in her breast. Her past medical history revealed AML, treated and considered in remission. A biopsy was performed in the palpable mass and the histopathological findings were consistent with a Granulocytic sarcoma of the breast, which was considered as a relapse of AML.
A wide spectrum of hepatobiliary manifestations occur in Human Immunodeficiency Virus (HIV)-infected patients. Among the common causes are the infectious hepatitis, drug-related hepatitis, opportunistic infections, non-alcoholic steatohepatitis, HIV cholangiopathy and neoplasm. Auto-immune hepatitis (AIH) is rarely reported in this setting. We present two different presentations of auto immune hepatitis in HIV positive patients. One developed jaundice and ascites as a consequence of liver decompensation and other exhibited cholestatic pattern. Their serology and liver biopsy confirmed autoimmune hepatitis as underlying aetiology. We would like to share the clinical improvement with simultaneous immunosuppressive therapy and combination Anti Retroviral Therapy (cART). There are no documented cases on this issue from the Indian subcontinent that we are aware of.
Adrenal cyst initially described in 1670s, is being diagnosed more frequently with the advent of widespread use of diagnostic imaging. Most of these cysts are small and remain nonfunctional with requirement of only conservative approach. Functional, malignant or benign lesions more than 5cm in size needs surgical intervention. Giant adrenal cysts (>10cm) do pose a clinical diagnostic dilemma and present with symptoms as was the scenario in the case being described. The management of this case along with a review of literature is being briefly described.
