La parálisis de Bell es una afectación periférica del séptimo par craneal por una causa desconocida. El cuadro se presenta de manera aguda y unilateral, donde se puede presentar dolor retro auricular de primera instancia y luego se presenta debilidad de los músculos faciales. El diagnóstico es clínico, en casos especiales se utilizan estudios complementarios y posee varios diagnósticos diferenciales, entre los más importantes se encuentran el accidente cerebrovascular, neoplasia y síndrome de Ramsay Hunt. El tratamiento se debe iniciar de forma aguda, siendo de elección los esteroides en las primeras 24 a 72 horas, asociado a terapia física. En más de un 90% de los casos con un buen pronóstico y sin secuelas a largo plazo.
Los senos venosos son los encargados de drenar la sangre cerebral hacia la vena yugular interna. La trombosis de estos senos es poco común y ocurre mayormente en mujeres de edad fértil o en estado de embarazo. La sintomatología que se produce en este cuadro clínico se debe al aumento de la presión intracraneal por lo cual lo más común son cefalea, papiledema y déficit motor. Entre las causas más comunes que producen esta patología están los estados protrombóticos y los cuadros infecciosos. El diagnostico se realiza mediante imagenología entre las cuales están la resonancia magnética y la tomografía axial computarizada. El tratamiento dependerá de la causa, si es un trastorno de coagulación, se darán anticoagulantes y si es por un cuadro infeccioso se cubrirá con antibióticos.
La hidrocefalia normotensa es una patología infrecuente y reversible, de etiología desconocida ya que las teorías presentadas no explican la sintomatología en su totalidad. Se clasifica como idiopática cuando se presenta el cuadro clínico usual de pacientes mayores de 70 años junto con la triada de Hakim clásica y las secundarias a alguna patología cerebral que se puede presentar a cualquier edad. El diagnóstico se basa en la clínica, medida de la presión media del líquido cefalorraquídeo y ventriculomegalia en imágenes radiológicas. Presenta una correlación con la enfermedad de Alzheimer y que no son excluyentes entre sí. Entre los diagnósticos diferenciales se encuentran las demencias e incontinencias urinarias usuales en edades avanzadas. La colocación de la válvula ventrículo peritoneal sigue siendo el tratamiento de primera línea.
La diverticulosis cólica se define por una anomalía anatómica adquirida del colon, que se caracteriza por la presencia de seudodivertículos formados por herniaciones de la mucosa y submucosa a través de la capa muscular del colon. Es una de las patologías más comunes a nivel del intestino grueso en el mundo occidental y su prevalencia durante el último siglo se ha incrementado probablemente debido a cambios en los estilos de vida. La enfermedad diverticular del colon es una condición común con presentación clínica variable y su presentación clínica abarca desde ser asintomática hasta la presencia de síntomas y el desarrollo de diversas complicaciones. Generalmente el diagnóstico se realiza de forma incidental, al realizar una colonoscopia o enema con medio de contraste. Los casos asintomáticos no requieren tratamiento y los pacientes sintomáticos se realiza modificaciones en la dieta y tratamiento oral. Las complicaciones requieren un manejo específico, ya sea con tratamiento antibiótico o quirúrgico.
