Background Globally, Tuberculosis (TB) is one of the top 10 causes of death. In Nepal, poverty and malnutrition aggravate the burden of TB. To identify Mycobacterium tuberculosis s putum is the best sample to identify the bacterium which is helpful for diagnosis. The aim of this study is to identify the situation, burden and challenges of pulmonary tuberculosis in low-middle income country like Nepal. Method A retrospective-audit with reliable-secondary-data of one year was collected (n = 4131). Descriptive-analysis was performed using frequency, percentage and analytical using chi-square-test. Level of significance was set at p < 0.05. Ethical Approval was obtained from IRC-PAHS. Result The prevalence of notified/suspected cases was highest among the patients having >60 years of age 1344(32.54%) and least among the patients with ≤15 years of age 239(5.79%). The male had 1.67 times more smear-positive pulmonary TB cases. Among the AFB-positive cases, smear 3+ was seen in most of the cases 69(38.54%) followed by smear 1+ and smear 2+ in 56(31.28%) and 54(30.16%) respectively. Conclusion The prevalence of smear-positive pulmonary TB case is higher in male. Smear 3+ is seen in most of the followed by smear 1+ and smear 2+. The Burden of Pulmonary TB is more among adult and old-age-people and its control is a challenge for developing and low-middle income countries like Nepal.
Introduction: COVID-19 has brought global challenges in the health and safety of people. It has created not only the economic and social disaster in global society but also pose the life of healthcare workers in danger. In such critical situation, health workers were the main fighters who stood against the spread of COVID-19 and were always involved in the care and treatment of COVID patients. The main objective of this study was to explore health workers’ motivational factors to work in COVID designated hospital during COVID-19 pandemic. Method: The study was conducted at Patan Hospital, Lalitpur. The study had collected the qualitative data from in-depth interview of doctors and nurses. Convenience sample method was used to collect the data. Total, 12 health workers (6 Doctors and 6 Nurses) were interviewed till thematic saturation. Collected information was transcribed and thematic analysis was done to analyze factors motivating health workers. Result: The study found three major factors that motivated the health workers: (i) Always remembering oneself as a Health worker (Doctor/Nurse), (ii) Commitment with the hospital and (iii) Own ethics that one has to serve. Conclusion: Despite difficulties and fear, the health workers were internally motivated to serve during the pandemic. However, they think they should be provided with the declared facilities, incentives and motivation plan to further encourage them.
Since the beginning of 2020 AD, the world is grappling with the COVID-19 pandemic and in Nepal, the first case was seen in the month of January itself. The second case, however, was detected only after 2 months and this gave the country an opportunity to upgrade the health system, especially for the pandemic. It also imposed a nationwide lockdown to contain the spread of the disease. Myagdi, a rural district of Nepal and its district hospital in Beni also prepared for the disease with the resources it had. The incoming route to the district was sealed off and people were asked to stay indoors unless there was an emergency. The regular OPD services of the hospital was also closed and a fever clinic was established where all the fever and suspected COVID-19 cases were seen. An isolation building was set up where the strong suspected cases were admitted and kept there until their PCR reports came out. Similarly, quarantine facilities were also made outside the hospital premises for those who had returned to the district from abroad. The hospital staffs were given Personal Protective Equipment, but they were scarce and had to be re-used. The whole scenario had appeared to put a mental toll on the hospital staffs as many of them were anxious and scared. At the same time, Dengue and Scrub Typhus cases also emerged in the district, adding to the health woes. However, the hospital staffs and the local bodies are stepping up to the challenge and are toiling hard to plan and execute for the containment of the disease in the district.
