Trigger finger is the commonest form of tendinopathy. In this study the comparison is done with two effective methods of management which is by giving steroid injection and percutaneous release.in this study two group were created of 40 patients in each group. Both the procedure was done in OPD day care basis. Patients were followed up to 6 months in gap of 1,3,6,12,24 weeks. The outcome was measured by VAS score and recurrence.
Inflow distortion is a problem encountered during axial fan/compressor operation. There are a large number of experimental studies that have been carried out to study the distortion effects on the axial fan/compressor performance by using wire mesh screens, by blocking inlet passage and physically disturbing the inlet flow. When the compressor experiences off design operation, it is likely to experience an inlet distortion, which determines its operational sustainability. Every compressor in each flight segment undergoes non-uniform inlet flow fields and responds differently to that of a clean inlet flow, for which it is designed. To meet specific performance requirements, it is often required for a compressor to pass through off-design environments. Distortion, simulated as waves or combination of waves in propagation, have been studied in this work. It has been observed that the effect of distortion depends on types of waves applied, area covered by distortion, operating speed of the fan/compressor & location of the distortion waves with respect to blade profile. For a given velocity in distorted passages spike wave has more detrimental effect compare to normal distortion wave or a triangular wave. A few combination distortion waves have been studied too.
We report a case of pentazocine induced multiple contractures leading to residual deformities in a young female presenting for emergency caesarean section. During preoperative assessment, severe disabling abduction deformities of both lower limbs at hip as well as knee and restricted movements of right upper limb were noted. History revealed intramuscular abuse of pentazocine and promethazine for 6 years, from an early age. She was presently de-addicted for last 3 years. Despite difficult positioning, we were able to perform the subarachnoid block successfully.
Background: Post burn neck contractures not only affect the movements of the neck, but also can affect the function of the lower face as well as result in possible tracheal alteration and distortion of the cervical spine. As these contractures cause major functional and cosmetic problems with resultant economic and psychosocial implications, operative correction is generally recommended, particularly in children in whom they can cause growth imbalance in the head and neck area. Aim: To determine and recommend the ideal skin cover for the post burn scar contracture release defects with regard to both functional and cosmetic outcome. Methods: Patients who were diagnosed as having post burn scar contracture of neck by clinical examination, were taken up for the study. Results: In our study, contracture release followed by skin grafting was done in 28(70%) patients, contracture release and flap cover and multiple z plasty in 6 (15%) each. 11 patients (27%) felt good, and 3 patients (11%) felt excellent on cosmetic appearance. Conclusion: TYPE A contracture, release and flap cover with supraclavicular flap is highly recommended and should be the first choice. For TYPE B contracture, Supraclavicular flap is not sufficient and has to substituted with a skin grafting. For TYPE C scar, release and skin cover with grafting is the ideal choice provided, proper post op care such as immobilization, positioning and splinting and compression therapy are followed up.
Air-cooled chillers are considered to be intensive energy consumers and account for over one-fourth of the energy consumption in air-conditioned buildings. In Dubai, for about 4-5 months, chillers operate at low part load ratios with a reduced coefficient of performance (COP) due to low outdoor temperatures. This paper evaluates the performance of air-cooled twin-circuit screw chillers serving a medium-scale commercial building in the month of January when the outdoor temperatures are low. The performance of the chillers is analysed in view of calculated parameters and optimal conditions obtained from the readings taken. The chillers are found to be operating poorly at low COPs. Measures to rectify the problems in the performance of the chillers and advanced chiller technologies that are currently under research are discussed.
