Os autores relatam o caso de um paciente que se apresentara com paraplegia flácida, distúrbios esfincterianos e alterações das sensibilidades superficiais, evoluindo no espaço de pouco mais de um ano para o resultado letal. No curso da doença o paciente apresentara oscilações térmicas que variavam de 37 a 38°, atingindo às vêzes 40°C. Surtos diarréicos entrecortavam ou substituíam às vêzes tenaz obstipação. Entre os vários exames de laboratório, assinalaram-se leucocitose com neutrofilia, presença de ovos de S. mansoni nas fezes. Foi efetuada terapêutica à base de antibióticos, vitaminas e radioterapia profunda, além de derivados de antimônio, sem resultado positivo. Na autópsia foram encontradas lesões granulomatosas fibrosantes esquistossomóticas localizadas nos cornos anteriores da medula, na comissura cinzenta e na substância branca dos funículos anteriores e laterais, com exclusão dos cordões posteriores. Os autores assinalam a concordância dos dados clínicos com os resultados do exame an átomo-patológico e discutem o problema do diagnóstico clínico e do trajeto dos ovos de S. mansoni, até chegarem à medula.

By the neurological, electromyographic, anatomopathological and bacterioscopic examinations of 100 pacients with leprosy the following conclusions have been drawn: There was a high incidence of reflex abnormalities. The proprioceptive reflexes were abnormal in 64.5% and the superficial reflexes in 70.8% of the cases. According to the topography of the segments examined, the deep reflexes showed the following abnormal values: 25.3% in the cephalic segment; 69.1% in the upper limbs and 74.5% in the lower limbs. Of the 2,289 reflexes investigated, abnormalities were found in 773 superficial reflexes (70.8%), while in relation to the proprioceptive reflexes the figures rose to 901 (64.5%). In the early stages of the infirmity, the reflexes were normal in 41.2% of the cases examined, and in all the stages of the evolution of the disease those numbers were always high. Also in the first stage, whose average duration was of 4.9 years, several alternations were noted, although there was a predominance of brisk reflexes (25.1%) over diminshed reflexes (17%), followed by hyperactive (8.6%) and absent reflexes (8%). In the final stage of the infirmity there was a decrease of normal reflexes, in contrast to absent and decreased reflexes, whose rates increased to 32.7% and 30.6% respectively. The disappearance of hyperactive reflexes and the clear decrease of brisk reflexes should also be noted. There was a high percentage of patients (31%) who suffered agression of the eighth nerve, 17 of which (54.8%) were found to suffer from neural deafness. The values related to hearing deficit were rarely in accordance with the histopathological and bacterioscopic positive results. The indexes of abnormalities found out in the examination of the fifth and of the seven nerve (7% and 5% respectively) were much lower in relation to the eighth nerve. These results are in contradiction with the ones stated in the literature. In some of the patients, hyperesthesia of the skin of the plantar region prognosticated the appearance of the infirmity. Among the objective changes, anesthesia of superficial sensibilities combined with deep hyperesthesia and anesthesia of superficial sensibilities with abolition of profound sensibilities were rarely observed. No case was found with exclusive alterations of deep sensibilities. Peripheral motor neuron lesions demonstrable by electromyography were observed in 82 patients (82%)...
As alterações do sistema nervoso na moléstia de Chagas experimental têm sido objeto de varias publicações, sendo particularizada a descrição de quadros histológicos (Kõberle 6 > 7 -8 > 9 e Brandão 10 ). Brandão avaliou quantitativamente os neurônios simpáticos e parassimpáticos no mal de Chagas experimental em sua fase crônica, observando haver "diminuição estatisticamente não significativa dos neurônios simpáticos dos ratos chagásicos, em confronto com uma redução numérica estatisticamente significante dos parassimpáticos". Kõberle pensa que tais alterações, verificadas na fase crô-nica da doença experimental, se instalam durante o período agudo, quando se decide o destino do paciente chagásico.Sabendo-se que o parasitismo antecede às alterações neuronals, nos propusemos analisar, em fase aguda, os plexos mientéricos do ponto de vista enzimático. MATERIAL E MÉTODOSDe um lote de 20 ratos albinos, pesando entre 25 e 30 g, inoculamos 10 com a cepa Y do Trypanosoma cruzi, na dose de 0,10 ml por via intraperitoneal, sendo os animais examinados cada 48 horas. O exame parasitêmico foi feito a fresco, entre lâmina e lamínula, com sangue retirado da cauda do animal. A parasitemia foi verificada a partir do 3' dia de inoculação, sendo encontrados 35 a 40 tripanosomas, em média, por campo microscópico.Os ratos infectados morreram na fase aguda da doença entre o 8" e 13' dias após a inoculação. Secções representativas do aparelho gastrintestinal (intestino delgado e cólon) foram fixadas em acetona a 4°C e formol tamponado à mesma temperatura: os espécimes fixados em formol foram impregnados pela prata, segundo variante do método de Gross-Bielschowsky, em cortes por congelação; segmentos intestinais fixados em acetona gelada foram utilizados para demonstração da atividade esterásica.Também foram feitas inclusões em parafina de material proveniente tanto dos arrimáis parasitados como dos animais controle, sendo as inclusões cortadas com espessura de 8 micra, em séries de 25 cortes sucessivos, totalizando uma espessura de 200 micra. Estes cortes foram corados pela hematoxilina-eosina e usados para estudo comparativo das possíveis alterações morfológicas entre os dois lotes.
