<b><i>Background:</i></b> Balloon compression of the gasserian ganglion has been a well-established percutaneous treatment of trigeminal neuralgia since the 1980s. However, puncture of the foramen ovale by conventional single-plane fluoroscopy can be difficult in cases of local anatomic abnormalities. <b><i>Case Presentation:</i></b> We present the case of a 49-year-old woman diagnosed with idiopathic trigeminal neuralgia refractory to pharmacological treatment. After failure of puncture by conventional fluoroscopy for percutaneous gasserian ganglion balloon compression due to a narrow foramen ovale, the patient was submitted to puncture guided by computed tomography. <b><i>Conclusion:</i></b> Alternative imaging methods, such as computed tomography, should be considered when puncture of the foramen ovale by conventional single-plane fluoroscopy fails, to minimize the risk of potential complications triggered by frustrated puncture attempts.
RESUMOO presente artigo -parte 2 de 3 -apresenta uma revisão sobre a abordagem diagnóstica do enfermo em estado de coma, destacando a investigação semiológica. Geralmente, a avaliação do indivíduo cujo nível de consciência encontra-se comprometido é uma tarefa complexa, tendo em vista as dificuldades para se analisar diretamente a consciência. Portanto, sua apreciação exige a observação atenta de um conjunto de sinais clínicos fundamentados em uma cuidadosa e sistemática avaliação neurológica a partir dos seguintes itens: (1) nível de consciência, (2) diâmetro e reflexo pupilar, (3) movimentação extrínseca do globo ocular, (4) padrão respiratório e (5) resposta motora esquelética aos diferentes estímulos. A adequada articulação entre estes itens, aliada aos dados coletados da anamnese -quando isto é possível -, permitirá a proposição de hipóteses diagnósticas, as quais guiarão a solicitação de exames complementares e a adoção de medidas terapêuticas.Unitermos. Generally, evaluating patients, whose level of consciousness is impaired, is a complex task, considering the difficulties to directly analyze the conscience status. Therefore, their evaluation requires a close observation of a set of clinical signs based on a careful and systematic neurological evaluation of the following aspects: (1) awareness, (2) diameter and pupillary reflex, (3) extrinsic movement of eyes, and (4) respiratory pattern, (5) skeletal motor response to different stimuli. The proper coordination between these items, together with data collected from the interview -when it is possible -will allow the proposal of diagnostic hypotheses, which will guide the requisition of additional tests and the therapeutic.
An 89-year-old woman admitted to the Emergency Care Department with reported back pain nine days earlier that started suddenly when sitting. This subsequently evolved into progressive paraparesis (Frankel D). An MRI of the lumbosacral spine revealed an extensive lumbar epidural hematoma ( Figure). Non-traumatic causes include hypertension, coagulopathies, use of anticoagulants, arteriovenous abnormalities, tumors of the backbone and lumbar puncture. RESUMOO estudo dos distúrbios da consciência tem como pressuposto a compreensão de como o sistema nervoso mantém o estado de vigília e a vida de relação. Além do estado de coma, outras situações igualmente graves podem ocorrer. Each of these disorders has specific characteristics distinguishing them, passing by the complete absence of consciousness during clinical examination -vegetative state -the perfect state of consciousness found in the locked-in syndrome.
A consciência é o perfeito estado do conhecimento de si próprio e do ambiente, bem como a responsividade específica a variados estímulos internos e externos. O presente artigo tem por objetivo apresentar os aspectos morfofuncionais, fisiopatológicos e as bases neurobiológicas das mais prevalentes condições mórbidas que envolvam alterações do nível de consciência. Para tal, foi realizada uma revisão da literatura apoiada em artigos obtidos através das bases LILACS (Literatura Latino-Americana e do Caribe em Ciências da Saúde), SciELO (Scientific Electronic Library Online) e PubMed (U. S. National Library of Medicine), além de livros-texto de neurologia e de neurociências. Neste primeiro artigo enfoca-se (1) a formação reticular – importante estrutura nervosa relacionada à consciência –, a qual é constituída por um conjunto de fibras nervosas originadas posteriormente ao tronco encefálico, que formam feixes ascendentes capazes de atingir o diencéfalo e ativar o telencéfalo. Distúrbios nestas áreas, de origem traumática ou não, podem levar ao comprometimento das funções tálamo-corticais e, consequentemente, a alterações do nível de consciência.
