BackgroundRecent guidelines on iron deficiency anaemia (IDA) have confirmed the aetiological role of Helicobacter pylori (H pylori), but the relationship still remains controversial.MethodsStarting in May 2009, searches of the following databases were undertaken: Medline (1966 to April 2009), Embase (1980 to April 2009), the Cochrane library (1800 to June 2008), Cochrane Central Register of Controlled Trials, Premedline, Healthstar, CBMdisc and the Chinese National Knowledge Infrastructure Database (January 1970 to April 2009). Changes in haemoglobin (Hb) concentrations and serum ferritin (SF) concentrations were recorded for intervention and control groups. The meta-analysis used random effect models and subgroup analyses were performed to explain heterogeneity.ResultsEight studies met the inclusion criteria. All studies were performed in Asia, an area with a high incidence of IDA and H pylori. The pooled analysis of eight studies showed that H pylori eradication therapy can improve IDA, since changes in Hb and SF concentrations in the intervention groups were higher than in controls. The weighted mean difference (WMD) of Hb was 12.88 g/l (95% CI 6.03 to 19.74 g/l, p<0.00001); the WMD of SF was 10.05 μg/l (95% CI 5.48 to 14.63 μg/l, p<0.00001).ConclusionsH pylori eradication therapy combined with iron administration is more effective than iron administration alone for the treatment of IDA. Eradication therapy has different effects on adults and children. Bismuth based triple therapy has a better response in terms of increased Hb and SF concentrations than proton pump inhibitor (PPI) based triple therapy.
The prognostic significance of ultrasound real-time elastography (RTE) in patients with breast lesions is controversial. There are two different diagnostic methods: the elasticity score (ES) and the strain ratio (SR). A meta-analysis was performed using a random-effect model to assess the overall sensitivity and specificity of RTE in the differentiation of breast lesions. MEDLINE, EMBASE, PubMed, and the Cochrane Library before February 2011 were searched. A total of 22 studies, which included 4,713 breast nodules in 4,266 patients were analyzed. The overall mean sensitivity and specificity for the diagnosis of malignant breast lesions by RTE were 0.834 [95% confidence interval (CI) 0.814-0.853] and 0.842 (95% CI, 0.829-0.854) for ES, and 0.883 (95% CI, 0.844-0.916) and 0.814 (95% CI, 0.786-0.839) for SR, respectively. RTE has a high sensitivity and specificity in the evaluation of breast lesions and can potentially reduce unnecessary breast biopsies.
CT is recommended, as it is an effective imaging tool for differential diagnosis in patients with primary SGT, and MRI is suggested for differential diagnosis between benign and malignant GSTs because of its highest sensitivity and specificity.
Aims: Renal fibrosis is a common outcome of chronic kidney disease. This study was designed to examine the protective effects of resveratrol (RSV) against renal fibrosis induced by unilateral ureteral obstruction (UUO). We also attempted to elucidate the potential mechanism involved. Methods: Mice were randomly divided into three groups: sham-operated, UUO, and UUO/RSV (20 mgÁkg
À1
Áday À1). Histological changes were examined using periodic acid-Schiff and Masson's trichrome staining after 14 days. Superoxide dismutase (SOD), malondialdehyde (MDA), and 8-OHdG levels were determined using a commercially available kit. ICAM-1, TNF-a, and TGF-b levels were measured using real-time PCR. Fibronectin levels were measured by western blot, and the Smad3 acetylation and Sirt1 were examined by immunoprecipitation and western blot. Results: Our study showed that RSV treatment significantly attenuated renal injury including extracellular matrix deposition and tubulointerstitium damage. Renal cortical mRNA levels of ICAM-1, TNF-a, and TGF-b, protein expression of fibronectin and Smad3 acetylation were significantly upregulated in the UUO group. However, treatment with RSV significantly decreased the expression of these proteins. Furthermore, RSV also decreased the levels of reactive oxygen species (ROS) including MDA and 8-OHdG, and increased the level of SOD, which protects cells against ROS damage. Conclusion: Our findings suggest that RSV treatment inhibits oxidative stress, Smad3 acetylation, and renal interstitial fibrosis. Therefore, RSV may have potential as a therapeutic target for the treatment of chronic kidney disease.
Objectives
The hemodynamics of the ultrasound (US) features of port‐wine stains (PWSs) have not been reported. The purpose of this study was to evaluate the high‐frequency US findings of PWSs.
Methods
We retrospectively reviewed 98 PWS cases and categorized them into 4 groups based on clinical manifestations and pathologic findings (14 nodular, 28 thickened, 52 flattened, and 4 acquired). We evaluated the US findings with 10‐ and 22‐MHz transducers.
Results
For nodular PWSs, the mean thickened skin ± SD was 0.6 ± 0.5 mm, and the nodule thickness was 5.7 ± 3.3 mm; 63.6% of skin lesions showed vessel density of 2.16 ± 0.93/cm2 with venous flow of 4.6 ± 1.1 cm/s, and all nodules showed vessel density of 6.14 ± 1.92/cm2 with arterial and venous flow of 26.6 ± 17.9 and 9.9 ± 5.1 cm/s, respectively. The thickened skin of the thickened type was 1.4 ± 2.7 mm; 76.5% of skin lesions showed vessel density of 3.81 ± 1.98/cm2 with venous flow 4.6 ± 2.1 cm/s. The thickened skin of 36 flattened lesions was 0.1 ± 0.1 mm; 91.7% of skin lesions showed vessel density of 1.08 ± 0.28/cm2. The thickened skin of 4 acquired lesions was 0.7 ± 0.4 mm; 50% showed vessel density of 1.08 ± 0.28/cm2. The thickened skin and vessel density of nodular and thickened PWSs were thicker and higher than those of the flattened ones. The 22‐MHz transducer produced clearer contrast and higher vessel density than the 10‐MHz transducer.
Conclusions
These results showed differences in US findings of PWSs, which may be useful for clinical diagnosis.
