In einer ffiiheren Arbeit wurden Untersuchungen fiber die Bedeutung des spezifischen Antitoxins ffir Krankheitsentstehung, Krankheitsschutz und natfirlichen Heilungsvorgang bei Diphtherie mitge~eilt. Das Ergebnis genannter Naehforschung konnte dahin zusammengefaBt werden, dab dem spezifischen Gegengift bei den Immunit~tsvorg~ngen der Diphtherie die wichtigste l~olle zuf~llt, wenn auch die Wirkung anderer Abwehrmechanismen, wie primiire Giftresistenz, Buctericidie usw. in manchen Fiillen nicht bestritten werden kann. Der Mangel an freiem Antitoxin im Verlaufe (l~osling, Siegl, Zoelch) und in der Rekonvaleszenz mancher unbehandelter diphtherischer Erkrankungen hat Zweifel an dem Wert einer Behandlung mit spezifischem, artffemdem Antitoxin aufkommen lassen. Verst~rkt wurden diese Bedenken an der Wirksamkeit der seit Behring gefibten Serumtherapie, als diese bei den geh~uft auftretenden hypertoxischen Diphtherien der ]etzten Jahre vielfach versagte. Extrem kommen solche Ansehauungen in den J~uBerungen Frledbergers zum Ausdruck, der die Grundlagen der Serumanwendung ftir ersehiittert h~lt: Bei dem ,,Versagen" der spezifischen Therapie sei Erforsehung und Auffindung .unspezifischer (bactericider) Mittel dringend notwendig.Wenn gesagt wurde, dal3 leichteste Diphtherien ohne Serum ausheilen, so gilt dies auch nur fiir einen kleinen Tefl der Fs Gerade die Ver6ffentlichungen Siegls zeigen, daB ein solches Vorgehen nur bei genauester kliniseher Beobachtung und gewisserma~en ,,mit bereitgehaltener Serumspritze" durchfiihrbar ist. Der genannte Autor beriehte~, dab yon 38 Dil0htherieerkrankungen die Hii]fte zur spontanen Heihmg kam, ,,w~hrend man sich bei den fibrigen 19 l~llen daim doch entschloB, die Serumbehandlung einzuleiten". Auch Feet kommt zu gleichen Ergebnissen. Selbst in den leichtesten l~gllen wird man besonders Untersuchungen fiber die Bedeutung des Antitoxins usw.
