Introdução: O lúpus eritematoso sistêmico (LES) é uma doença inflamatória crônica autoimune multissistêmica. Alguns pacientes, contudo, podem exibir um padrão histológico de lesão renal, com características indistinguíveis da nefrite lúpica, porém sem apresentar quaisquer sintomas extrarrenais ou sorologias sugestivas de LES. Tal acometimento tem sido recentemente denominado nefropatia full-house não relacionada ao lúpus. Descrição: P.M.G., 19 anos, feminino, solteira. Apresentou quadro de síndrome edemigênica associada à hipertensão com proteinúria em faixa nefrótica e alterações de função renal em exames laboratoriais. Foi investigada quanto à possibilidade de LES sem preencher critérios clínicos e laboratoriais. Durante a internação foi submetida à biópsia renal com padrão compatível com nefropatia full-house sendo realizada pulsoterapia com Ciclofosfamida com recuperação total da função renal e, atualmente, mantem acompanhamento em uso de corticoterapia. Conclusões: A Nefropatia full-house não relacionada ao lúpus é uma entidade nova, ainda não totalmente compreendida em seus mecanismos de ação e abordagem terapêutica. Ainda necessita de mais estudos e pode se apresentar como uma manifestação inicial do LES, manifestação isolada do LES ou uma doença nova não relacionada ao LES.
