Nang đường tiêu hóa đôi là một dị dạng bẩm sinh hiếm gặp, thường có một lớp niêm mạc phủ trung lòng nang. Nang dạ dày đôi chiếm khoảng từ 2 - 9% trong tổng số nang đường tiêu hóa đôi. Biểu hiện lâm sàng đặc trưng là đau vùng thượng vị, nôn và sờ thấy khối ổ bụng. Chẩn đoán xác định dựa vào siêu âm và chụp cắt lớp ổ bụng. Phương pháp điều trị là phẫu thuật cắt bỏ nang. Chúng tôi báo cáo trường hợp của 1 bé trai 8 tháng tuổi có biểu hiện nôn, chậm lên cân, được chẩn đoán trào ngược dạ dày thực quản tại tuyến trước không kết quả và được chuyển Bệnh viện Nhi Trung ương. Được làm siêu âm và chụp CT ổ bụng với hình ảnh nang dạ dày đôi. Bệnh nhân được phẫu thuật nội soi cắt nang. Sau phẫu thuật trẻ diễn biến ổn định được bú sữa mẹ sau 24h sau mổ và xuất viện sau ngày thứ 3 sau phẫu thuật. Nang dạ dày đôi dễ nhầm với luồng trào ngược dạ dày thực quản, hẹp phì đại cơ môn vị. Do vậy, khám lâm sàng và chẩn đoán hình ảnh giúp xác định chẩn đoán chính xác nhất. Phẫu thuật cắt bỏ nang là phương pháp điều trị duy nhất.
Phẫu thuật nội soi một đường rạch (PTNSMĐR) có nhiều ưu điểm song ít được áp dụng ở trẻ em. Chúng tôi đánh giá kết quả áp dụng phẫu thuật nội soi một đường rạch trong điều trị bệnh Hirschsprung ở trẻ sơ sinh. Nghiên cứu mô tả tiến cứu trên các bệnh nhi Hirschsprung từ 1-28 ngày tuổi được phẫu thuật nội soi một đường rạch tại Bệnh viện Nhi Trung ương từ tháng 01/2020 đến tháng 12/2021. Kết quả có 23 bệnh nhi, tuổi trung bình 22,3±3,2 ngày (17-28 ngày). Thời gian mổ trung bình 53,8±11,9 phút (35-75 phút), thời gian hậu phẫu trung bình 4,5±1,1 ngày (3-7 ngày). Có 4 trường hợp viêm ruột, 1 hẹp miệng nối, không có tử vong. Nghiên cứu này cho thấy phẫu thuật nội soi một đường rạch an toàn và hiệu quả trong điều trị bệnh Hirschsprung ở trẻ sơ sinh.
Ghép gan được coi là biện pháp điều trị cuối cùng cho các bệnh lý gan giai đoạn cuối, suy gan cấp và một số khối u ở gan, cho cả đối tượng trẻ em và người lớn. Sự khan hiếm về nguồn tạng ghép đã làm cho việc ghép gan từ người cho sống trở nên ngày càng phổ biến. Chúng tôi tiến hành hồi cứu trên 31 cặp ghép gan cho trẻ em được tiến hành tại bệnh viện Nhi trung ương từ tháng 7/2018 - 3/2023 nhằm đánh giá tính án toàn của phẫu thuật trên nhóm người cho gan. Hệ thống phân loại theo Clavien - Dindo được áp dụng để đánh giá mức độ các biến chứng gặp phải. Trong số 31 người cho gan có 15 nam, 16 nữ, tuổi trung bình 33,74. Bố mẹ ruột hiến gan cho con chiếm 67,8%. Các loại mảnh ghép được lấy bao gồm: thùy bên trái 26 trường hợp (87,1%), gan phải 3 (12,9%), gan trái 2 (6,5%). Thời gian phẫu thuật trung bình 301,8 phút, thời gian nằm viện trung bình 9,1 ngày. Bất thường sau mổ gặp ở 9 trường hợp, trong đó 8 phân loại mức độ nhẹ, 1 mức độ nặng (từ độ III trở lên) theo Clavien-Dindo, không có trường hợp nào tử vong. Từ kết quả nghiên cứu cho thấy, phẫu thuật lấy mảnh ghép gan từ người cho sống với người nhận trẻ em có thể được thực hiện an toàn với tỉ lệ biến chứng thấp.
Trực tràng đôi là một bệnh lý rất hiếm gặp với các triệu chứng lâm sàng nghèo nàn, không điển hình. Do đó, việc chẩn đoán và điều trị còn gặp nhiều khó khăn. Chúng tôi báo cáo một ca bệnh trực tràng đôi với bệnh nhân nam 1 tháng tuổi, siêu âm trước sinh không phát hiện gì đặc biệt. Trẻ vào viện vì không đại tiện 10 ngày. Khám hậu môn thấy một khối ở vị trí 9 giờ với kích thước khoảng 3cm, chắc, bề mặt nhẵn, ranh giới đẩy lồi thành trực tràng vào trong, gây hẹp lòng trực tràng. Trên phim cộng hưởng từ thấy khối nang dịch kích thước 3x4x5cm sát thành sau trực tràng, xuất phát từ ống tiêu hóa. Bệnh nhân được chẩn đoán nang trực tràng đôi và được tiến hành phẫu thuật nội soi ổ bụng kết hợp đường hậu môn cắt đoạn trực tràng chứa nang trực tràng đôi. Theo dõi 3 tháng sau mổ thấy bệnh nhân có sức khỏe tốt, đại tiện bình thường, không thấy tái phát. Trực tràng đôi là một bệnh lý hiếm gặp với triệu chứng lâm sàng đa dạng và không điển hình, cần chẩn đoán và điều trị kịp thời để tránh các biến chứng. Phẫu thuật là điều trị triệt để.
