A diffúz nagy B-sejtes lymphoma (DLBCL) a leggyakoribb malignus lymphomatípus, amely a non-Hodgkin-lymphomák (NHL) 31%-át teszi ki. A standard kemoterápiás protokollok jelentős toxicitásuk miatt nem vagy nehezen alkalmazhatóak az idősebb betegpopulációban, különösen a relabált esetekben, ahol kuratív lehetőséget leginkább a nagy dózisú kezelés és a vér-őssejtátültetés jelenthetne. Egyre több adat lát napvilágot a thalidomid és az újabb IMiD-ek (lenalidomid, pomalidomid) kedvező hatásairól az NHL kezelésében, amelyek részei a standard kezelésnek myeloma multiplexben és myelodysplasiás szindrómában. Az IMiD-ek kiterjedt anti-angiogenetikus, immunmoduláns és gyulladásellenes hatásuknak köszönhetik az egyre széleskörűbb alkalmazást. 2010-ben azonosították az E3-ubiquitin-ligáz-komplex egyik komponensét, a cereblont, amely a thalidomid molekuláris támadási pontja. Az irodalomban közölt eredmények alapján thalidomidkezelést indítottunk három relabált, idős DLBCL-es betegünknél. Két esetben központi idegrendszeri, egy esetben abdominalis érintettség volt jelen. A betegek napi 100 mg thalidomidterápiában részesültek kortikoszteroidkezeléssel kiegészítve. Két beteg kiválóan reagált a kezelésre, tartós remisszióba került, amit képalkotó vizsgálattal is igazoltunk, és ezek a betegek 12, illetve 20 hónappal a kezelés elindítása után is remisszióban vannak. Egy beteg KIR-érintettséggel a kezelés ellenére progrediált és elhunyt. Az irodalmi adatoknak megfelelően a thalidomid és analógjai szignifikáns aktivitást mutatnak relabált DLBCL-ben. Esettanulmányunk egy olyan betegpopulációban elért ígéretes eredményeket mutat be, amelyben a túlélési esély igen alacsony. Emiatt, valamint költséghatékonysága és alacsony toxicitása miatt a thalidomidkezelést érdemes idős betegeknél terápiarefrakter és relabált malignus DLBCL esetében megfontolni. In elderly patients treatment is problematic because of the high toxicity of standard chemotherapy protocols, especially in relapsed cases, where high-dose chemotherapy and haematopoietic stem cell transplantation would be the best choice. More and more data is becoming available on alternative treatment of refractory/relapsed NHL, including studies on the positive effect of thalidomide and second generation IMiDs in DLBCL, which are already part of the standard treatment protocol in myeloma multiplex and myelodysplasia. The broadening use of IMiDs is due to their anti-angiogenetic, immunmodulatory and anti-inflammatory properties. In addition, a component of the E3-ubiquitin ligase complex, named cereblon, has been described in 2010 as the molecular effector of the thalidomide signal transduction pathway.
