Personality disorders are considered a possible factor affecting the relationship between therapeutic alliance and therapy outcome. The present study investigated the alliance-outcome effect in patient groups with borderline personality disorder (BPD) and obsessive-compulsive personality disorder (OCPD). Data derived from a sample of n = 66 patients, treated in a day care hospital setting with a dialectical-behavioral and schema therapeutic treatment concept. Patients rated their symptom severity at admission, early alliance after 4–6 therapy sessions and symptom severity as well as alliance at discharge. Results showed no significant differences between BPD and OCPD patients regarding symptom severity and alliance. Multiple regression analyses indicated that the alliance was a significant predictor of symptom reduction, however only in the OCPD group. Our results showed an exceptionally strong alliance-outcome relationship in OCPD patients, suggesting that focusing on building a strong alliance and measuring it early in therapy may be especially beneficial for this patient group. For patients with BPD, a more regular screening of the therapeutic alliance might be helpful.
Hintergrund: Angststörungen sind häufig, oft chronifizierend, jedoch auch gut behandelbar. Leitlinienbehandlung ist die auf Exposition fokussierende Verhaltenstherapie. In der ambulanten Versorgungspraxis finden Expositionen jedoch selten statt. Die Angst-Tagesklinik am Universitätsklinikum Dresden realisiert die evidenzbasierte Behandlung von Angststörungen. In einer 5-wöchigen Kurzzeitbehandlung werden wöchentlich 4 begleitete und zeitoffene Expositionssitzungen durchgeführt. Untersucht wurden die Symptomverläufe und die Responder-Raten. Methoden: Zu Therapieaufnahme und -ende sowie zur Katamnese nach 3 Monaten und 1 Jahr wurde die Entwicklung der Symptombelastung bei n = 332 PatientInnen untersucht, die von 2009 bis 2015 behandelt wurden. Etwa zwei Drittel waren psychotherapeutisch vorbehandelt. Eingesetzt wurden spezifische etablierte Fragebögen. Berechnet wurden Mixed Models, Effektstärken und Responder-Raten. Ergebnisse: 90% der PatientInnen schlossen die Behandlung regulär ab. Es zeigten sich signifikante Linderungen bei Belastungen durch Angst- und depressive Symptome. Die höchsten Effektstärken um 0,9 ergaben sich bei verhaltensbezogenen Skalen und besonders bei den Agoraphobien und Panikstörungen, die die größte Störungsgruppe ausmachen. Die Responseraten lagen bei 60%. Zu den Katamnesezeitpunkten waren die Symptomverbesserungen stabil und bei den kognitiven Symptomen weiter steigend. Schlussfolgerungen: Die Behandlung in spezialisierten (teil)stationären Versorgungssettings mit Fokus auf hochfrequente Exposition, wie hier beispielhaft vorgestellt, zeigt eine gute Akzeptanz und gute bis sehr gute und längerfristig stabile Veränderungen der Symptombelastung. Diese Settings erfordern eine besondere strukturelle Ausstattung und Ressourcen.
Objectives: The present naturalistic study aims to investigate the differential effects of the patient’s and the therapist’s attachment representations on the attachment to the therapist as perceived by the patient, and their impact on self-esteem-change through psychotherapy.Methods: Attachment variables of N = 573 patients as well as N = 16 therapists were assessed. Attachment representations were measured for therapists and patients via the Bielefelder Questionnaire for Client Attachment Exploration, the Relationship Specific Attachment to Therapist Scales and the Adult Attachment Interview. The patient’s attachment to therapists was evaluated and patients’ self-esteem was measured via the Frankfurter Selbstkonzeptskalen at the beginning and end of psychotherapy.Results: Although there were significant effects of the patient’s attachment representations on the perceived attachment to the therapist as well as between the perceived attachment to the therapist and the amount of self-esteem-change, the therapist’s attachment style had no significant influence on the perceived attachment to the therapist.Conclusion: Self-esteem-change through psychotherapy is influenced by the actually formed attachment relationship as perceived by the patient. The patient’s attachment representations but not the therapist’s attachment style contributes to the actual patient’s attachment to the therapist.
Zusammenfassung Ziel der Studie Die therapeutische Beziehung gilt als bedeutsamer und empirisch gut gesicherter Einflussfaktor auf den Psychotherapieerfolg. Ziel der vorliegenden Studie war es, diesen Effekt anhand einer großen tagesklinischen Stichprobe zu replizieren sowie verschiedene Einschätzungsbereiche der therapeutischen Beziehung in einer Extremgruppe besonders niedriger Beziehungszufriedenheit gesondert zu betrachten. Methodik Es erfolgte eine longitudinale Betrachtung von n=809 Patienten (MW=34,32; SD=10,7; 72,6% weiblich) in einem tagesklinischen Versorgungssetting. Der Zusammenhang von therapeutischer Beziehung (Helping Alliance Questionnaire; HAQ-S) in der dritten Behandlungswoche und Therapieerfolg (Brief Symptom Inventory-18; BSI-18) wurde mittels multipler Regressionanalysen berechnet. Dies erfolgte sowohl für die Gesamtstichprobe, als auch die Extremgruppe des unteren Dezils der Fälle mit der geringsten Beziehungszufriedenheit sowie der übrigen 90 % der Fälle. Hierbei wurde zwischen Beziehungs- und Erfolgszufriedenheit als Subskalen des HAQ differenziert. Ergebnisse Die therapeutische Beziehung nach 3 Wochen war ein signifikanter Prädiktor von Therapieerfolg. In der Extremgruppe des Dezils mit der initial niedrigsten Beziehungszufriedenheit zeigte sich dieser Zusammenhang als statistisch signifikant und stark, jedoch aufgrund des breiten Konfidenzintervalls nicht praktikabel zur Prädiktion individueller Fälle (β=0,622; 95% KI [0,051; 1,095]). Dagegen ergab sich für die übrigen 90% der Fälle für die Beziehungszufriedenheit keine über die Aufklärung durch die Erfolgszufriedenheit (β=0,244; 95% KI [0,176; 0,391]) hinausgehende Varianzaufklärung beim Therapieerfolg. Diskussion Die Ergebnisse bestätigen die Bedeutung des Wirkfaktors der therapeutischen Beziehung auch in einem tagesklinischen Setting. Der Beziehungszufriedenheit kommt nur in der Extremgruppe besonders unzufriedener Patienten eine zentrale, andere Faktoren übersteigende Bedeutung in der Prädiktion des Therapieerfolges zu. Schlussfolgerung Die Sicherstellung einer zumindest ausreichend guten therapeutischen Beziehung ist von großer Bedeutung und Bedarf daher der frühzeitigen Identifizierung und gegebenenfalls Intervention bei besonders ungünstig verlaufender therapeutischer Beziehung.
