BackgroundThe problem of long-stay hospitalization is still a pressing issue. In this study we examined the possibility of detecting and characterising the group at risk of long-stay hospitalization in advance.MethodsThis study examines the data of patients in the urban catchment area of the Medical University of Hannover, capital of Lower Saxony, Germany, during a period of 10 years.Results and conclusionThe introduced "psychosocial risk-score", calculated at the first institutional contact, was able to predict the risk of long-term hospitalization. Characteristics of social disintegration, especially with regard to employment status, are of particular importance.
The following review concludes the authors experience of a 6 year consultation with the parents of a 28 year old daughter with a schizophrenic psychosis. The self-sacrificing experiences of the parents are reported as well as the experiences with a well developed system of psychiatric care with several professionals, whose work remains without success because of insufficient coordination. The author asks for a conference of all involved persons, to nominate a responsible therapeutic key-worker to coordinate the different psychiatric care in the future.
Einleitung Die Sozialpsychiatrische Zusatzausbildung an der MHH Die sozialpsychiatrische Zusatzausbildung (SPZA) an der Medizinischen Hochschule Hannover (MHH) existiert seit 1969 (www.mh-hannover.de/kliniken/sozialpsychiatrie/spza/html) [1 -3]. Die SPZA ist eine 2-jährige berufsübergreifende und institutionsübergreifende psychiatrische Fortbildung mit 720 h, die seit 1972 mit einem staatlich anerkannten Abschlusszeugnis des Niedersächsischen Landesamtes für zentrale soziale Aufgaben abschließt. Die Teilnehmer kommen aus ambulanten, teilstationären und stationären psychiatrischen Einrichtungen der Region Hannover, die für 1,1 Millionen Einwohner 90 Anbieter psychiatrischer Hilfen zur Verfügung hat. Von 1971 -2005 haben 556 Personen die SPZA absolviert, davon kommen 67 % aus psychiatrischen Kliniken, 33 % aus ambulanten, teilstationären und komplementären psychiatrischen Einrichtungen. Die Berufsgruppe Krankenpflege betrug 63 %, Sozialarbeit 20 %, andere Berufe 17 % (Altenpflege, Ergotherapie, Krankengymnastik, Pädagogik, Kunsttherapie, Handwerk). Die Kursgröße der jährlich beginnenden Fortbildungsmaß-nahme umfasst 18 Plätze, bisherige Kurse hatten 1971 -2005 eine durchschnittliche Größe von 16 Personen, davon 63 % weiblich und 27 % männlich.
