Purpose: To discuss which brain region potentially functioned and switched between the immediate and delayed response of acupuncture. Materials and Methods:A nonrepeated event-related functional MRI (fMRI) design was used to investigate the spatial and temporal patterns of acupuncture effects induced by needling an acupoint ST36 (ACU) and a nonmeridian point (SHAM). The standard general linear model was used to detect the immediate neural responses of acupuncture. Graph theory analysis was used to characterize the functional integrated network of the acupuncture delayed effect.Results: Acupuncture induced significant signal changes in the limbic/paralimbic areas, neocortical regions, brainstem, and cerebellum for immediate effect both in ACU and SHAM. Some of these brain regions showed strong functional connectivity for a delayed effect in ACU. Conjunction analysis showed that the insula played a critical role during the overall process of ACU. No overlapping brain regions were found in SHAM. Conclusion:The findings of this study suggested that the delayed effects may reflect a more significant characteristic underlying acupuncture. Given that the insula as a relay station switched between the immediate and delayed response, it suggested that divergent functional connectivity patterns may mediate the acupuncture-related effects for ACU and SHAM.

Purpose Here we present our experience with five cerebral proliferative angiopathy (CPA) patients to better delineate the clinical and angiographic features as well as the treatment selection of this disease. Methods Between October 2008 and October 2012, five consecutive patients diagnosed with CPA were admitted to our department in our hospital. All the five patients received magnetic resonance imaging, digital subtraction angiography, and positron emission computed tomography (PET) to definitively confirm this disease. We also collected 15 previously published instances of CPA to analyze the characteristics of this rare entity. Results As to the five patients, three were female and two were male, between the ages of 4 and 52 years with a mean age of 24.8 ± 20.6 years. The PET results showed that perfusion was decreased over the affected hemispheres in all five patients. As to the treatment, only one patient received encephalo-duro-arterio-synangiosis (EDAS) revascularization surgery. The other four patients were conservatively observed. During the follow-up period (range 3–6 years, mean 4 ± 1.9 years), the patient who underwent EDAS surgery manifested relief of clinical symptoms. In the conservative series, the symptoms in two patients aggravated and suffered permanent neurologic deficits. Conclusion CPA is a rare entity. Natural history has showed this disease is not stable and may progress at a certain time point. The EDAS procedure may be a treatment for CPA-related oligemia since there is currently little data and follow-up available.

This study indicates that there are two main clusters of arterioles in the distal second dorsal metacarpal artery that can be helpful for the second dorsal metacarpal artery pedicle cutaneous chain-link perforator flaps in the repair of defects in the fingers.

Objective: Immune cells are key mediators of secondary damage following spinal cord injury (SCI), and dendritic cell (DC)-based vaccines have received considerable interest for treatment of SCI. We previously showed that vaccination with DCs pulsed with homogenate proteins of the spinal cord (hpDCs) promotes functional recovery from SCI in mice. However, the underlying molecular mechanisms remain unclear. Here, changes of neurotrophins, cytokines and T cells at the site of SCI in mice after vaccination with hpDCs were investigated and correlated with recovery from SCI. Methods: hpDCs, DCs (control) or PBS (control) were injected intraperitoneally into injured mouse spinal cords. Functional recovery of the spinal cord was measured weekly using the Basso Mouse Scale (BMS) and confirmed by histological and immunohistochemical analysis. Brain-derived neurotrophic factor (BDNF), neurotrophin-3 (NT-3), interleukin-4 (IL-4) and interferon-γ (IFN-γ) levels in T cell culture supernatants and spinal cord tissues were determined by ELISA. Results: Eighty-four days after immunization, the BMS score of the hpDCs group (6.92 ± 0.20) was significantly higher than those of the DCs and PBS groups (p < 0.01). Meanwhile, the injury area and number of cysts in the hpDCs group decreased significantly compared with control groups. BDNF, NT-3, IL-4 and IFN-γ levels at the injured site as well as BDNF and NT-3 levels in the supernatant of cultured T cells from the hpDCs group were significantly higher than in control groups (p < 0.05). Conclusion: These results reveal that vaccination with hpDCs can promote SCI repair potentially by upregulating BDNF, NT-3, IL-4 and IFN-γ at the injury site.
