Case:
A 37-year-old man presented after a sudden snap followed by pain in the medial side of the ankle while playing football. Physical examination showed edema and pain posteriorly to the medial malleolus, and a longitudinal structure was palpable anteriorly. Ultrasound confirmed a dislocated tibialis posterior tendon (TPT). The patient underwent surgery, and the flexor retinaculum was repaired. Six months after the surgery, he was asymptomatic and had resumed his usual physical activity, with an AOFAS score of 100.
Conclusion:
TPT dislocation is rare and orthopaedic surgeons should be aware of this condition in patients with a sudden traumatic medial ankle pain.
Переломы бедра -весьма распространенное явление среди взрослых, однако достаточно редкое у де-тей (состав ляют менее 1 % от всех переломов у детей). В статье представлен клинический случай межвер-тельного перелома бедра IV типа (по Delbet) со смещением у 6-летней девочки, лечение которого проводи-лось путем установки скользящего бедренного винта. Металлофиксаторы были удалены через 10 мес. после операции. Переломы бедра IV типа (по Delbet) составляют около 12 % от всех переломов бедра у детей. У детей старше 3 лет подобные переломы требуют выполнения остеосинтеза скользящим бедренным винтом или проксималь-ной бедренной блокируемой пластиной. Репозицию желательно проводить в течение 24 часов после перелома. Несмотря на то что в нашем случае операция была выполнена не сразу, восстановление пациентки проходило хорошо, без каких-либо описанных в литературе осложнений в течение периода наблюдения, который прод-лился 26 мес. после удаления импланта.Ключевые слова: травматология; ребенок; переломы бедра; остеосинтез.
The distal radius is the third most common site of giant cell tumor of bone (GCTB). The local aggressive invasion of this rare neoplasm requires reconstructive solutions after wide excision. The authors present two cases of patients diagnosed with Campanacci grade III GCTB of the distal radius successfully treated with en-bloc excision and translocation of the ipsilateral ulna. Pre-operative application of denosumab was given for one year to both patients. At one year of follow-up, both patients are disease-free and reported satisfactory results on Quick - Disabilities of the Arm, Shoulder and Hand (Quick-DASH) questionnaire and modified Musculoskeletal Tumor Society (MSTS) score. Although a challenge, the reported procedure offers good oncological and functional outcomes. Merkel Cell Carcinoma is a rare type of aggressive and often fatal neuroendocrine tumor of the skin.We report a clinical case of a 56-year-old female with a Merkel cell carcinoma on the left hand, without distant metastasis, treated with surgical excision of the lesion and radiotherapy.Based on this clinical case a research and review of the literature on this rare neoplasia was carried out.
