La sarcoidosis es una enfermedad multisistémica que afecta a cualquier órgano, raramente al corazón. El objetivo de este trabajo es presentar el caso de un paciente con muerte súbita por sarcoidosis, lo cual es infrecuente, aunque aproximadamente el 25% de los pacientes tienen granulomas no calcificados en diferentes órganos, pero sólo el 5% tienen enfermedades cardíacas. El caso clínico presenta a un paciente de 43 años con hipertensión arterial severa con tratamiento especializado y con varios ingresos por insuficiencia coronaria aguda. En el último, es trasladado a nuestra sala por infarto en evolución. Encontramos solamente una radiografía de tórax con adenopatías hiliares bilaterales, confirmado por tomografía axial, reportándose además adenopatías mediastínicas. El resto de los exámenes eran normales, incluyendo ultrasonido abdominal. El electrocardiograma mostró hipertrofia ventricular izquierda, isquemia anterior extensa, fibrilación auricular y extrasístoles ventriculares. Presentó parada cardíaca con muerte súbita. La necropsia mostró sarcoidosis pulmonar, ganglionar e infiltración endomiocardíaca. Se ha demostrado que esta enfermedad, cuando afecta el corazón, tiene un pronóstico ominoso por las diferentes alteraciones cardíacas que ocasiona, pero lo más terrible es la muerte súbita, como ocurrió en este caso.
