OBJETIVO: Em resposta à necessidade sentida por municípios brasileiros de informação referente às práticas de amamentação com vistas ao planejamento em saúde, realizou-se estudo com o objetivo de descrever a situação da amamentação e identificar fatores associados ao desmame nesses municípios. MÉTODOS: De uma convocação aberta a todos os municípios do Estado de São Paulo, 84 aderiram a um treinamento para coleta de dados no Dia Nacional de Vacinação, em 1998. Para cada município, foi desenhada uma amostra compatível com o tamanho de sua população infantil, aplicando-se um questionário padronizado com questões referentes à alimentação da criança nas 24 horas precedentes. Além de estatísticas descritivas sobre a freqüência de amamentação, foram analisados pela regressão logística fatores de risco para interrupção da amamentação exclusiva em menores de quatro meses e para o desmame em menores de um ano. RESULTADOS: O aleitamento exclusivo nos primeiros quatro meses raramente alcançou índices superiores a 30%. Como fatores de risco para essa situação, identificaram-se: baixa escolaridade materna, ausência de programa Hospital Amigo da Criança, primiparidade e maternidade precoce. Com relação aos menores de um ano, a amamentação ficou em torno de 50%. CONCLUSÃO: A ausência do programa Hospital Amigo da Criança, primiparidade, trabalho informal ou desemprego materno foram os fatores de risco para o desmame. As taxas municipais de amamentação diferem amplamente no Estado de São Paulo, o que reforça a importância de diagnósticos locais, rápidos e de fácil apropriação por profissionais de saúde.

This paper describes rapid assessment of infant feeding practices based on surveys conducted on National Immunization Day in the cities of Florianópolis and João Pessoa, Brazil. Two different infant feeding patterns emerge clearly in the data analysis. Most infants begin breastfeeding, but exclusive breastfeeding (EBF) from 0-4 months (46.3% in Florianópolis and 23.9% in João Pessoa) and timely complementary feeding rates (32.2% in Florianópolis and 24.8% in João Pessoa) are below recommended standards. EBF and breastfeeding duration medians were 53 and 238 days, respectively, in Florianópolis and 16.5 and 195 days, respectively, in João Pessoa. The results pointed to increasing breastfeeding rates and duration medians in Florianópolis as compared to João Pessoa. Use of these data could improve planning and monitoring of breastfeeding activities and infant nutrition policies.
Prevalence of overweight and obesity in elderly people from Vitória-ES, Brazil Prevalência de sobrepeso e obesidade em idosos da cidade de Vitória-ES, BrasilResumo O objetivo deste estudo foi avaliar a prevalência de sobrepeso e de obesidade associados a fatores socioeconômicos, em uma amostra de idosos não institucionalizados de Vitória-ES, Brasil. Foi realizado um estudo transversal com uma amostra de 882 idosos com 60 anos ou mais. Estes parâmetros foram avaliados por meio do índice de massa corporal (IMC) e da circunferência da cintura (CC). Todos os participantes responderam a um questionário de dados pessoais e sociodemográ-ficos em relação à idade, sexo, grau de instrução, estado civil, atividade física, número de filhos, doenças crônicas e tabagismo. Associações entre as variáveis categóricas foram testadas utilizando o teste qui-quadrado com um nível de significância 5%. Observou-se que prevalência de sobrepeso e obesidade de acordo com o IMC foi alta (41,8% e 23,4%, respectivamente) e 50,7% dos idosos apresentaram um substancial aumento da circunferência abdominal. Verificou-se que 4,3% dos indivíduos eram diabéticos, 50,4% hipertensos e 14,9% apresentaram ambas as doenças. Observou-se que tanto o IMC como a CC foram significativamente associados (p <0,05) com o sexo, estado civil, presença de doenças e tabagismo. Palavras-chave Estado nutricional, Idoso, Obesidade, Sobrepeso Abstract The scope of this study was to estimate the prevalence of overweight and obesity and its association with socio-economic status in a sample of non-institutionalized elderly people from Vitória-ES, Brazil. This was a cross-sectional survey with a sample of 882 elderly people aged 60 and over. Obesity and overweight were assessed using the body mass index (BMI) and waist circumference (WC). All subjects answered a personal and socio-demographic questionnaire in relation to age, gender, marital status, physical activity, number of children, chronic diseases and smoking. Associations between categorical variables were tested using chi-square analysis with a 5% significance level. The prevalence of overweight and obesity was high (41.8% and 23.4%, respectively) and 50.7% of the elderly had a substantially increased waist circumference. About 4.3% of the individuals had diabetes, 50.4% had hypertension and 14.9% were found to have both diseases. It was observed that both the BMI and WC were significant associated (p<0.05) with sex, marital status, the presence of diseases and with cigarette smoking.
