Introdução: Este estudo objetivou descrever as alterações hematológicas presentes em gestantes com COVID-19 descritas na literatura. Métodos: Foram realizadas buscas nas bases de dados SCOPUS, MEDLINE, LILACS e EMBASE. Foram selecionados quinze artigos, que atenderam aos critérios de inclusão. A amostra foi composta por uma maioria de relatos de casos (53,3%) e apenas um estudo prospectivo (6%). Resultados: Entre as alterações evidenciadas nos estudos, destacou-se a elevação do D-dímero, trombocitopenia, PCR elevada, além de hiperfibrinogenemia ou hipofibrinogenemia. Verificou-se, ainda, aumento importante nos níveis de TTPA nas mulheres infectadas. Em alguns estudos, não foram notadas alterações significativas entre gestantes com e sem a doença, observando-se, em especial, gestantes com quadros leves ou moderados da infecção. As complicações relacionadas às alterações hematológicas descritas foram o tromboembolismo pulmonar, trombose da artéria esplênica e sangramento intraoperatório anormal. Destacou-se a possibilidade de ajuste na anticoagulação profilática, além da necessidade de vigilância rigorosa a essas mulheres.Conclusão: Por fim, ressaltou-se a importância da realização de outros estudos mais aprofundados, que permitam estudar melhor a associação dessas alterações com a infecção pelo SARS-CoV-2.
