Supervised training periods in primary care have been used as spaces for teaching and extension in the area of health, making it feasible to include undergraduates in concrete teaching-learning scenarios. The aim of this study was to analyze the perceptions of dental students about the importance of supervised training periods in Family Health Units to their professional education. The sample consisted of 185 students who answered the question: What is your opinion about the importance of this training period in SUS to your professional education? Comment on this experience and its positive and negative aspects The responses were analyzed by the quali-quantitative Collective Subject Discourse (CSD) technique. The students appreciated learning through practice in the service; contact with professionals from other areas; opportunity for technical-operative improvement and demonstrated sensitivity in the face of social reality, although they appeared to be concerned about being absent from the faculty, arguing that they were being prejudiced as regards their intramural clinical productivity, exhaustively demanded of them. It was concluded that students placed value on the extramural experience, however, it was perceived that there was still a predominant influence of focus on intramural clinical training.
O estágio extramuros é uma importante estratégia pedagógica utilizada na formação das profissões da saúde no sentido de fomentar uma prática mais integral e compreensiva do ponto de vista social, sobretudo quando desenvolvido em cenários reais de serviços públicos do Sistema Único de Saúde. Essas experiências nem sempre são bem aceitas pelos alunos, de forma que explorar os fatores que interferem nessa valoração pode ser útil para aprimorar essa atividade. O objetivo do presente estudo foi explorar quais percepções dos alunos de um curso de Odontologia influenciam a valoração a respeito da importância do estágio em Unidades de Saúde da Família na sua formação profissional. Um total de 185 alunos que realizaram estágio durante seus últimos anos de graduação participaram da pesquisa. Os dados foram obtidos por meio de formulários eletrônicos compostos por questões sobre o funcionamento e condições de trabalho nas unidades de saúde (variáveis independentes) e pela valoração de importância atribuída ao estágio (variável dependente). Os alunos que perceberam falta de material de consumo (OR= 2,17, IC= 1,20 - 4,00), limpeza e higienização incompatíveis com a prestação de serviços de saúde (OR= 7,69, = 2,32 – 25,00) e a falta de respostas satisfatórias por parte dos trabalhadores das equipes de saúde aos usuários (OR= 4,34, IC= 1,63 - 11,11) tiveram maiores chances de considerar a experiência como não-importante. Concluiu-se que problemas infraestruturais e aporias do cotidiano dos serviços parecem influenciar numa valoração negativa do estágio por parte dos alunos, apontando a necessidade de um maior apoio do corpo docente para transformar essa leitura da realidade em produtos pedagógicos importantes aos alunos
Resumo Objetivou-se compreender com maior grau de aproximação aspectos da relação entre a odontologia e a sociedade por meio da observação direta do cotidiano e do discurso de dentistas membros de equipes da Estratégia Saúde da Família, que discorreram sobre sua terapêutica na área da saúde bucal num novo cenário normativo. Estudo desenvolvido utilizando o Método Clínico-Qualitativo durante o ano de 2016, que partiu do tema “terapêutica da cárie dentária” como assunto disparador para o desenvolvimento das entrevistas, realizadas por meio de roteiro semidirigido de questões abertas em profundidade, nas próprias unidades de saúde. O material obtido foi analisado por meio de Análise Qualitativa de Conteúdo. Os resultados evidenciaram configurações de poder provenientes de um modelo odontológico conservador, desfavoráveis ao propósito da Promoção da Saúde, mas apontaram também potencialidades trazidas pelo modelo de Saúde da Família, reveladas por meio da criatividade e reflexividade sobre ações educativas e desafios que puderam estimular o profissional. Considerou-se, entretanto, que o trabalho do dentista nesse contexto conservou, predominantemente, um modus operandi hegemônico, caracterizado por relações desequilibradas de poder.
The article presents a critical review regarding the premature termination of eating disorder's treatment among inpatients and outpatients, with the aim of identifying-emotional, psychodynamic and family-related aspects involved. The search strategy used the following MeSH terms combined by Boolean operators: "eating disorders" AND "treatment" OR "patient dropouts" OR "drop-out/dropouts" OR "attrition" OR "premature termination" AND "empirical study" OR "qualitative research", for on Medline/PUBMED, PsycINFO and EMBASE databases. This article follows the PRISMA Guidelines. A total of 26 studies composed this review, of which 24 were original research articles, 1 was a review and 1 a theoretical article. Only two articles applied qualitative methods analyzing categories of content obtained by in-depth interviews, three combine quantitative and qualitative methods and other three present qualitative analyses while discussing quantitative studies. Further qualitative studies should be carried out to clarify meanings of dropout, premature termination of treatment, and attrition. Different expectations held by patients and by therapeutic teams, and the interpersonal difficulties of these types of patients, stand out as difficulties in constructing "therapeutic alliances", with impacts on dropout, premature termination and attrition rates.
The aim of this study was to analyze the value and meanings that dental surgeons attribute to the Primary Health Care setting, where health promotion is encouraged over a mechanistic performance of procedures. A qualitative study, involving ten Brazilian dental surgeons working in Primary Care in 2016, was designed. In-depth semi-structured interviews were performed, with all interviews recorded, transcribed and subsequently submitted to Qualitative Content Analysis. Despite the Healthcare Promotion model proposed by the Brazilian oral health policy, dental surgeons demonstrated preferences for private and traditional dental practices. These characteristics are counterproductive in public oral health services, which aim to achieve collective health benefits. Traditional practice is based upon a specific and restricted focus, as opposed to overall patient care, hence maintaining the original professional identity, ruled by manual procedures, while demonstrating scientifically fragile understanding of disease processes. Despite the implementation of public service models that aim at change, counterproductive characteristics associated with the deeply rooted traditional management strategies were evidenced.
Objectives: to analyze the understanding of the Therapeutic Itinerary of people affected by stroke in the Unified Health System context of a medium-sized city in the state of São Paulo. Methods: this is a clinical-qualitative, using in-depth semi-structured interviews, conducted in 2019 with 6 patients, a number determined by theoretical saturation. Results: qualitative content analysis allowed the construction of four categories that emerged from the data: a) You are having a stroke! - Identifying the problem; b) Anguish and expectations in waiting for assistance; c) Helplessness and guidance for rehabilitation after hospital discharge; d) Spirituality in rehabilitation after stroke. Final Considerations: the affected people expressed feelings and meanings, such as fear, anguish, sadness, beliefs and uncertainties, in order to shed light on the complexity inherent to the experiences of Therapeutic Itinerary of stroke.
Introduction: Basic and daily oral hygiene care is essential for maintaining oral health in adults with special needs. The caregivers act in this process and need to be understood about the difficulties and the resources they use to deal with this demand. The aim of this qualitative study was to understand the difficulties and strategies of caregivers regarding oral hygiene for adults with special health care needs (SHCN)Methods: Twenty-one caregivers of adults with SHCN participated in an interview in which the following questions were asked: What are the greatest difficulties in performing oral hygiene for the adult with SHCN you take care? and "How do you overcome these difficulties?" The interviews were recorded and the qualitative data were analyzed using the Discourse of the Collective Subject technique Results: All respondents were female with a mean age of 58.5 years (± 10.8) and the adults with SHCN presented mean age of 30 years (± 18). The greatest difficulty pointed out by caregivers regarding oral hygiene for adults with SHCN was their non-cooperation to do such activity. Regarding coping strategies, caregivers pointed out several strategies to perform oral hygiene in adults with SHCN, such as supervising their brushing, seek for a qualified dentist to assist with this task, performing oral hygiene during the bath, performing physical restraint, among others Conclusion:The caregivers' perspectives indicate feelings of failure, discomfort, but also the use of creative resources to deal with the task. The results bring up themes that refer to anxieties and are related to technical and dental issues, that could be explored and need to be recognized by the reference oral healthcare team, whose responsibility it is also to work for the empowerment of caregivers. RESUMO Buscou-se investigar e conhecer a frequência e a distribuição de frenotomias e fenectomias realizadas no Sistema Único de Saúde (SUS) após os dispositivos legais que instituíram o teste da linguinha, bem como entender quais profissionais estão realizando o procedimento cirúrgico. Estudo descritivo analítico, retrospectivo, com dados secundários, disponíveis no Sistema de Informações Ambulatoriais do DataSUS, extraídos por meio do TabWin. Observou-se um aumento considerável no número de procedimentos, porém, com flutuações nos anos seguintes. Das frenectomias registradas no SUS, no período, 33% foram realizadas por dentistas na atenção primária. Os resultados apontaram que houve um aumento substancial no número de procedimentos cirúrgicos em dois momentos, em 2014 e em 2017, possivelmente em decorrência da publicação da Lei nº 13.002/2014 e da Nota Técnica (NT) nº 09/2016. A NT nº 35/2018 deslocou o diagnóstico e o tratamento para a média complexidade, possivelmente resultando em filas, principalmente em se tratando de cirurgias que poderiam ser realizadas na atenção primária. Apesar de não haver unanimidade quanto à correlação anquiloglossia e desmame, não parece haver um monitoramento para evitar cirurgias desnecessárias, uma vez que a ordenha pode não ser afetada e o acompanhamento ser realizado para confirmar ou não essa necessidade.
