Leitthema Zervixkarzinom ist weltweit die zweithäufigste bösartige Tumor erkrankung der Frau [1]. Im Jahr 2004 erkrankten in Deutschland 6190 Frau en [2]. Die Inzidenz ist seit der Einfüh rung des zytologischen Abstrichs in die klinische Routine rückläufig und liegt seit Mitte der 1980erJahre an nähernd stabil bei 15/100.000 Frau en. In jüngster Zeit nährt die Impfung gegen Subtypen des humanen Papil lomavirus die Hoffnung, dass sich die Zahl der Neuerkrankungen des Zer vixkarzinoms weiter reduzieren lässt. Inwiefern dies zutrifft, bleibt abzu warten. Bis dahin ist die operative Therapie -aber auch die RadioChe moTherapie -ein wichtiger Pfeiler in der Behandlung der Erkrankung.
Leitthema NOTES (Natural Orifice Transluminal Endoscopic Surgery) ist eine Weiterentwicklung der minimal-invasiven Chirurgie, die als endoskopische Zugangswege natürliche Körperöff-nungen, wie den Gastrointestinaltrakt oder die Vagina, nutzt. Im Gegensatz zur klassischen Laparoskopie ergibt sich so die Möglichkeit einer Operation ohne sichtbare Narben. Dadurch wird die Invasivität im Vergleich zur klassischen Laparoskopie noch weiter gesenkt. Die Technik ist fester Bestandteil unseres operativen Spektrums, insbesondere bei der Adnexektomie, und soll im Folgenden dargestellt werden.
