Second generation triazoles are widely used as first-line drugs for the treatment of invasive fungal infections, including aspergillosis and candidiasis. This class, along with itraconazole, voriconazole, posaconazole, and isavuconazole, is characterized by a broad range of activity, however, individual drugs vary considerably in safety, tolerability, pharmacokinetics profiles, and interactions with concomitant medications. The interaction may be encountered on the absorption, distribution, metabolism, and elimination (ADME) step. All triazoles as inhibitors or substrates of CYP isoenzymes can often interact with many drugs, which may result in the change of the activity of the drug and cause serious side effects. Drugs of this class should be used with caution with other agents, and an understanding of their pharmacokinetic profile, safety, and drug-drug interaction profiles is important to provide effective antifungal therapy. The manuscript reviews significant drug interactions of azoles with other medications, as well as with food. The PubMed and Google Scholar bases were searched to collect the literature data. The interactions with anticonvulsants, antibiotics, statins, kinase inhibitors, proton pump inhibitors, non-nucleoside reverse transcriptase inhibitors, opioid analgesics, benzodiazepines, cardiac glycosides, nonsteroidal anti-inflammatory drugs, immunosuppressants, antipsychotics, corticosteroids, biguanides, and anticoagulants are presented. We also paid attention to possible interactions with drugs during experimental therapies for the treatment of COVID-19.
Cel pracy: W pracy przedstawiono zastosowanie metod oceny wieku zębowego z wykorzystaniem zdjęć rentgenowskich dla celów medyczno-sądowych. Zaprezentowano różne przypadki praktycznego zastosowania opisywanych metod do identyfikacji szczątków ludzkich, oceny wieku zębowego u osoby żywej oraz jeden przypadek archeologiczny. Materiał i metody: Materiał badawczy stanowiły przypadki oceny wieku zębowego przeprowadzonej w Katedrze i Zakładzie Medycyny Sądowej Uniwersytetu Medycznego im. Karola Marcinkowskiego w Poznaniu. W zależności od wstępnej oceny wieku zastosowano metodę Liversidge lub Kvaal i wsp. Oceny wieku zębowego dokonano na podstawie dostępnych zdjęć pantomograficznych. W przypadku osoby żyjącej było to zdjęcie dostarczone do ekspertyzy, w pozostałych przypadkach wykonano tomografię komputerową stomatologiczną. Wyniki: We wszystkich przypadkach ustalono wiek zębowy. Wykorzystano różne metody oparte na analizie zdjęć rentgenowskich. Wiek zębowy wykazywał korelację z wiekiem kostnym. Wnioski: Wykazano, że omawiane metody, w których wykorzystuje się zdjęcia rentgenowskie, są skuteczne, a uzyskane wyniki w pełni korelują z wynikami badań antropologicznych.
StreszczenieWstęp: Ocena stanu zdrowia skazanych na karę ograniczenia wolności stanowi istotny odsetek spraw opiniowanych rocznie w Katedrze i Zakładzie Medycyny Sądowej Uniwersytetu Medycznego im. Karola Marcinkowskiego w Poznaniu. Pozwoliło to na dokonanie analizy wydanych opinii i wskazanie trudności, jakie mogą napotkać biegli lekarze. Materiał i metody: Analizie poddano 1051 opinii sądowo-lekarskich dotyczących oceny stanu zdrowia skazanych na pracę na cele społeczne, wydanych w latach 2004-2013. Z opinii uzyskano dane dotyczące płci, wieku i rodzaju schorzeń skazanych oraz wniosków biegłych w zakresie zdolności do pracy społecznie użytecznej. Wyniki: Wśród skazanych przeważającą większość stanowili mężczyźni w wieku 51-60 lat. Najliczniejszą grupę stanowiły osoby zdolne do pracy społecznie użytecznej po spełnieniu określonych warunków, wskazanych w opinii biegłych (56,3%), najmniej liczna była grupa osób uznanych za zdolne do wykonywania pracy bez jakichkolwiek ograniczeń (9,5%). W przypadku skazanych uznanych za zdolnych do pracy jedynie po spełnieniu wskazanych przez biegłych warunków najczęściej zalecano pracę lekką (58,4%). Najliczniejszą grupę schorzeń skazanych stanowiły choroby narządu ruchu (51,4%), drugą w kolejności -schorzenia układu krążenia (38,7%). Wnioski: Liczba spraw dotyczących zdolności do odbywania kary ograniczenia wolności, opiniowanych rocznie w Katedrze i Zakładzie Medycyny Sądowej Uniwersytetu Medycznego im. Karola Marcinkowskiego w Poznaniu, oraz liczba wyroków skazujących w skali kraju wskazują, że biegli lekarze mogą otrzymać tego rodzaju sprawy do zaopiniowania. Główną trudnością w ocenie zdolności skazanego do wykonywania pracy na cele społeczne jest brak informacji, jakiego rodzaju i na jakim konkretnie stanowisku osoba skazana miałaby wykonywać pracę w ramach kary ograniczenia wolności. Objectives: A rare case of a suicidal use of gases from the car exhaust system of the car petrol engine in an open space is presented. Clinical Presentation and Intervention: To verify suicidal intent of poisoning, an experimental reconstruction of the crime scene was performed. This demonstrated that appropriately long exposure to carbon monoxide with unfavorable weather conditions can cause acute carbon monoxide poisoning in an open space. Conclusions: This case was studied to observe the pattern of fatal carbon monoxide poisoning in an open space. This might be helpful for forensic medicine, emergency medicine, and occupational medicine.
