OBJETIVO: Os autores apresentam os resultados de reconstruções esofágicas com retalho microcirúrgico de jejuno obtidos no Instituto Nacional de Câncer entre dezembro de 1998 e março de 2001, ressaltando suas vantagens, desvantagens e suas complicações. MÉTODO: Foram estudados sete pacientes portadores de neoplasias avançadas de laringe, faringe ou da tireóide que foram submetidos no Instituto Nacional de Câncer (Rio de Janeiro-Brasil) à reconstrução esofágica cervical com interposição de retalho livre de jejuno após faringolaringectomia associada à esofagectomia cervical. Foram cinco homens e duas mulheres com uma média de idade de 54 anos (39-66). Três pacientes foram submetidos à reconstrução esofágica imediata e em quatro casos a reconstrução foi tardia, em média após 10,5 meses. RESULTADOS: Não existiram perdas do retalho ou mortes no período pós-operatório avaliado. Das complicações, a mais freqüente foi infecção de ferida operatória (57,1%). Fístulas salivares acometeram dois casos (28,5%) e necessitaram de tratamento cirúrgico. Outras complicações incluíram estenose (14,1%) e hipopafatireoidismo (28,5%). Não houve complicações abdominais. Quatro dos sete casos apresentaram recidiva da doença em um período médio de 16,6 meses. Cinco pacientes reconquistaram uma satisfatória capacidade de deglutição após o procedimento. CONCLUSÃO: A experiência apresentada sugere que o retalho livre de jejuno consiste em um procedimento seguro, com elevados índices de sucesso para reabilitação da via digestiva e com baixos índices de complicações se comparado a outros métodos.
Foram analisados 253 casos de reconstrução nasal, 80,63% devido a carcinoma basocelular e 7,51% devido a carcinoma espinocelular. As principais áreas comprometidas foram: dorso (41,50%), asa (30,43%), ponta (24,90%) e parede lateral (12,25%). Excluindo-se o avançamento direto simples, o principal procedimento utilizado na reconstrução do dorso foi o retalho dorso-glabelar (17,14%); da asa nasal, o retalho nasogeniano (55,84%); da ponta, o retalho bilobado (20,63%) e da parede lateral, o retalho nasogeniano (58,06%). Para os casos de reconstrução total de nariz (2,77%) o procedimento mais utilizado foi o retalho de Converse (71,42%).
Research has shown that cells from adult fat tissue can effect long-term blood reconstitution. Fat-derived multipotentiality was ascribed to CD34<sup>+</sup> perivascular populations from its prominent microvasculature, that represent mostly non-hemogenic, mesenchymal cells, although this tissue contains a CD34<sup>+</sup>45<sup>+</sup> subset committed to a hemogenic fate. Here, in order to analyze cell subsets presenting hemogenic capabilities within fat, CD133/1<sup>+</sup> and pericytes, the latter defined by CD140b (PDGFRb, Platelet-Derived Growth Factor Receptor Beta) expression, were immunomagnetically selected from stromal-vascular fractions (SVF). <i>In Vitro</i> Colony Forming Unit (CFU) assays were negative for CD140b<sup>+</sup> pericytes and positive for CD133/1<sup>+</sup> cells when a prolonged CFU assay was performed, revealing fat as another store of primitive progenitors that retain hemogenic potential
