A ruptura do septo interventricular (RSI) é uma das três complicações potencialmente letais do infarto agudo do miocárdio (IAM), incluindo ruptura da parede do ventrículo esquerdo (VE) e regurgitação mitral aguda. Geralmente, estão associadas à instabilidade hemodinâmica e elevada mortalidade, mesmo quando os reparos cirúrgico ou intervencionista são realizados a tempo. O reparo cirúrgico consiste no tratamento definitivo e a sobrevida ocorre somente após intervenção cirúrgica. Todavia, trata-se de uma terapêutica desafiadora e associada a alta mortalidade intra-hospitalar. Apesar de altamente fatal, a RSI é um evento relativamente raro, dessa forma, ainda não estão disponíveis diretrizes objetivas para o seu adequado manejo. Neste relato de caso, descrevemos a primeira paciente com RSI pós-IAM extenso e complicado que teve sucesso no reparo cirúrgico, na unidade de terapia intensiva coronariana (UCO) do Hospital de Base do Distrito Federal (HBDF), um hospital terciário da Secretaria de Saúde do Distrito Federal (SED-DF).
