INTRODUÇÃOA síndrome de Aicardi foi descrita em 1965 por Aicardi e col. Representa 1 a 4% dos casos de espasmos infantis e ocorre provavelmente devido a mutação ligada ao X. Encontrada quase que exclusivamente em meninas, contudo já foram relatados dois casos em meninos, que apresentavam um cromossomo X extra (XXY), e somente um caso familial é conhecido (1)(2)(3) .Achados característicos da síndrome incluem espasmos infantis, agenesia de corpo caloso e lacunas coriorretinianas (tríade característica), freqüentemente associados com coloboma do disco óptico. As lacunas coriorretinianas são os achados patognomônicos da síndrome, e associadas às anormalidades na ressonância magnética (heterotopia periventricular, córtex displásico, cistos ependimais), permitem o diagnóstico. Anormalidades vertebrocostais estão presentes em metade dos casos (2,(4)(5)(6) .As lesões características do fundo de olho são lacunas brancas, circulares, bem definidas, com pigmentação mínima de suas bordas, variando em tamanho de 1/10 a 20 diâmetros papilares. Usualmente são bilaterais e com freqüência distribuídas simetricamente. Em geral as lesões estão agrupadas em torno do disco óptico e diminuem em tamanho e número quando se estendem para a periferia (3) .Histologicamente existem múltiplas áreas de heterotopia e polimicrogiria das camadas do cérebro, enquanto as lacunas representam um afinamento do epitélio pigmentar e coróide, com perda de grânulos de pigmento. Cistos ependimais são freqüentemente encontrados próximo ao terceiro ventrículo e cistos ou tumores do plexo coroidal podem atingir grandes tamanhos. Eles podem permitir um diagnóstico pré-natal quando encontrados com agenesia de corpo caloso (2,(6)(7) .Em casos raros o corpo caloso pode estar presente. O prognóstico é ruim, com persistência das crises epiléticas e retardamento acentuado. A morte ocorre nos primeiros anos de vida (6,8) . Apresenta alterações neurológicas e oftalmológicas características que incluem espasmos infantis, agenesia de corpo caloso e lacunas coriorretinianas (tríade característica), freqüentemente associados com coloboma do disco óptico. Para fetos masculinos a morte ocorre intra-útero, e para meninas, nos primeiros anos de vida. O prognóstico é ruim, com persistência das crises epiléticas e retardamento grave. Descrevemos os casos de duas pacientes com essa síndrome atendidas em nosso serviço. O exame oftalmológico foi decisivo em ambos os casos, já que as alterações características encontradas, associadas à agenesia do corpo caloso fecham o diagnóstico da síndrome.
