Nada a declarar RESUMO: Existe uma grande variedade de instrumentos utilizados para a avaliação da força de preensão manual. Porém, não existem estudos demonstrando a validade e a confiabilidade da maioria destes instrumentos. O objetivo deste estudo foi avaliar a validade concorrente e a confiabilidade intraexaminador do dinamômetro Saehan hidráulico comparado-o com o dinamômetro Jamar hidráulico. Cem indivíduos sadios (50 homens e 50 mulheres), entre 20 e 50 anos de idade, sem alterações cognitivas, deficiências físicas, disfunções neuromusculares e ortopédicas e história de lesões nos membros superiores, foram testados com os dinamômetros Jamar e Saehan. A validade concorrente entre o dinamômetro Jamar e o dinamômetro Saehan foi excelente para os testes de força de preensão realizados com as mãos direita (r=0,976) e esquerda (r=0,986). A confiabilidade intra-examinador foi excelente tanto para o dinamômetro Jamar (r=0,985 mãos direita e esquerda) quanto para o dinamômetro Saehan (r=0,981 mão direita e r=0,985 mão esquerda). O dinamômetro Saehan é válido, confiável e comparável com o dinamômetro Jamar. Portanto, dados coletados com o dinamômetro Jamar são equivalentes aos dados coletados com o dinamômetro Saehan. Consequentemente, valores de força obtidos por testes com dinamômetro Saehan podem ser comparados com valores de referência de força de preensão que foram obtidos com o dinamômetro Jamar. There is, however, a lack of studies showing the validity and reliability of most of these instruments. The purpose of this study was to examine the concurrent validity and test-retest reliability of the hydraulic Saehan dynamometer using the hydraulic Jamar dynamometer. One hundred healthy subjects (50 men and 50 women), between the ages of 20 and 50 years old, without cognitive impairment, physical disability, neuromuscular or orthopedic dysfunction, and history of upper extremity injuries were tested with the Jamar and Saehan dynamometers. The concurrent validity between the Jamar and the Saehan dynamometers was excellent for both the right hand (r=0.976) and the left hand (r=0.986). Test-retest reliability was excellent for both the Jamar (r=0.985 right and left hands) and the Saehan (r=0.981 right hand and r=0.985 left hand). The Saehan dynamometer is valid, reliable and comparable to the Jamar dynamometer. Therefore, the Jamar and Saehan dynamometers measure handgrip strength equivalently. As a result, strength values obtained with the Saehan dynamometer can be compared with reference values which were collected with the Jamar dynamometer.
As quedas em idosos são um problema frequente, com importantes consequências físicas, psicológicas e sociais. O objetivo deste estudo foi realizar uma revisão sistematizada da literatura sobre as consequências das quedas em idosos vivendo na comunidade. A busca foi realizada entre junho a outubro de 2008 nas bases de dados eletrônicas Medline, Lilacs, PEDro e SciELO. Apenas artigos em inglês, português e espanhol foram considerados para esta revisão. Foram identificados inicialmente 114 artigos potencialmente relevantes. Após análise do título e dos resumos, considerando-se critérios de inclusão e exclusão predeterminados, foram selecionados para esta revisão dez artigos. Foram identificadas as seguintes consequências: fraturas, imobilização, lesões de tecidos moles, contusões, entorses, feridas e abrasões, lesões musculares e neurológicas, surgimento de outras doenças, dor, declínio funcional e da atividade física, atendimento médico, hospitalização, reabilitação, medo de cair, abandono de atividades, tristeza, mudança na vida/comportamentos, sentimento de impotência, declínio em atividade social, perda de autonomia e da independência, mudança de domicílio/ ambiente, rearranjo familiar e morte. As fraturas e o medo de uma nova queda ficaram dentre as consequências mais citadas. O conhecimento das consequências físicas, psicológicas e sociais das quedas em idosos é de extrema importância, pois ele auxiliará no delineamento das estratégias preventivas e de reabilitação de tais repercussões.
Falls are a major concern in the elderly population with chronic joint disease. To compare muscular function and functional mobility among older women with knee osteoarthritis with and without a history of falls, 15 elderly women with a history of falls (74.20 ± 4.46 years) and 15 without a history of falls (71.73 ± 4.73 years) were studied. Muscular function, at the angular speed of 60, 120, and 180º/s, was evaluated using the Biodex Isokinetic Dynamometer. The sit-to-stand task was performed using the Balance Master System and the Timed Up and Go test was used to determine functional mobility. After collection of these data, the history of falls was investigated. A statistically significant difference was detected in the time taken to transfer the center of gravity during the sit-to-stand test (means ± SD; non-fallers: 0.35 ± 0.16 s; fallers: 0.55 ± 0.32 s; P = 0.049, Student ttest) and in the Timed Up and Go test (medians; non-fallers: 10.08 s; fallers: 11.59 s; P = 0.038, Mann-Whitney U-test). The results indicated that elderly osteoarthritic women with a history of falls presented altered functional mobility and needed more time to transfer the center of gravity in the sit-to-stand test. It is important to implement strategies to guarantee a better functional performance of elderly patients to reduce fall risks.
A síndrome da fragilidade é uma condição associada ao envelhecimento, com desfechos de saúde nos idosos como quedas, medo de cair e incapacidade. Os objetivos deste estudo foram determinar a freqüência de fragilidade e verificar a existência de correlação desta com quedas, medo de cair e funcionalidade, em 30 idosos (média de 75,7±7,6 anos) cadastrados em um serviço interdisciplinar de Geriatria e Gerontologia. Foram avaliados quanto à fragilidade, por características sugeridas pela literatura, e quanto aos desfechos quedas, funcionalidade e medo de cair. A análise descritiva identificou 20% de idosos frágeis, 46,7% pré-frágeis e 33,3% não-frágeis. Foram encontradas diferenças significativas entre os grupos frágeis e pré-frágeis e frágeis e não-frágeis em relação à funcionalidade mensurada pela escala de Lawton (p=0,000) e medo de cair, avaliada pela escala internacional de eficácia de quedas (FES-I, na sigla em inglês). Foi encontrada correlação significativa e moderada entre a pontuação na FES-I e o número de quedas; e correlação significativa, moderada e inversa, entre as pontuações da FES-I e de Lawton. A freqüência de fragilidade foi maior na amostra do estudo do que a encontrada em estudos prévios; e foram encontradas diferenças significativas entre os grupos de idosos, de acordo com as características de fragilidade, indicando que os mais frágeis apresentavam maior incapacidade para atividades de vida diária e mais medo de cair.
OBJETIVO: Realizar uma revisão sistematizada da literatura sobre o impacto da prática regular de atividade física na qualidade de vida dos idosos. MÉTODO: Revisão sistemática de estudos publicados até setembro de 2008, nas bases de dados Medline, LILACS e SciELO. Foram excluídos os estudos que possuíam delineamento transversal ou que eram relatos de casos, que avaliavam a qualidade de vida associada a alguma doença específica e aqueles nos quais havia presença de outra intervenção concomitante. RESULTADOS: De acordo com os critérios de inclusão, dos 391 artigos encontrados, apenas três fizeram parte da análise. Foram identificados três tipos de intervenções: 1) exercícios de aquecimento, treino aeróbico, fortalecimento muscular, coordenação e resfriamento; 2) exercícios de fortalecimento muscular, treino funcional; 3) exercícios na água. Os resultados desses estudos sugerem que a qualidade de vida aumenta com a prática de atividade física (intervenções 1 e 3), principalmente se for realizada duas vezes por semana ao invés de somente uma vez. Os benefícios do treino de resistência muscular na qualidade de vida foram limitados e não foi encontrado benefício do treinamento funcional neste desfecho. CONCLUSÃO: O número limitado de artigos encontrados, apenas um único estudo para cada tipo de intervenção, limita as conclusões desta revisão a respeito da eficácia dessas intervenções. Assim, conclui-se que há evidência limitada a respeito dos benefícios da prática de atividade física na qualidade de vida de idosos vivendo na comunidade. Sugere-se que mais estudos longitudinais sejam realizados para que as conclusões a respeito da eficácia destas intervenções sejam mais robustas.
