As anomalias das artérias coronárias são malformações congênitas raras que, na maioria das vezes, não manifestam sintomas, mas se constituem a segunda causa de morte súbita entre jovens atletas. A origem anômala da Artéria Coronária Direita (ACD) é a mais prevalente, sendo o trajeto interarterial o mais frequentemente associado a morte súbita em até 30% desses casos. O objetivo deste artigo é relatar um caso de origem da ACD do seio aórtico esquerdo em um paciente jovem que procurou o pronto atendimento de um hospital terciário, com um quadro de síndrome coronariana aguda com supradesnivelamento do seguimento ST. Neste cenário, propõe-se descrever as formas de apresentação das origens anômalas da ACD, os principais exames complementares e o tratamento mais indicado. A metodologia utilizada foi a revisão de literatura acerca do tema, junto com a descrição do relato de caso em questão. O tratamento definitivo das anomalias das artérias coronárias com origem no seio aórtico contralateral é a cirurgia, indicada tanto aos pacientes sintomáticos quanto para os assintomáticos, uma vez que há grande probabilidade de morte súbita nesta população.
Introdução: O Infarto Agudo do Miocárdio (IAM) é uma das principais causas de óbito no Brasil e no mundo; de acordo o DATASUS de 2017, 7.06% dos óbitos no Brasil foram atribuídos ao IAM. Dentre os principais fatores de risco para o IAM, está a dislipidemia, relacionada principalmente a níveis elevados de colesterol de lipoproteína de baixa densidade (LDL-c). A dislipidemia ou hiperlipidemia refere-se ao aumento do colesterol total, LDL-c, triglicerídeos ou lipoproteínas, já a hipercolesterolemia refere-se ao aumento do LDL-c. As dislipidemias, na maioria das vezes, ocorrem etiologicamente por causas secundárias e fatores ambientais, e a hipercolesterolemia, comumente tem origem primária associada a distúrbios genéticos do metabolismo das lipoproteínas. As desordens hereditárias do metabolismo do LDL-c podem ser divididas didaticamente em patologias monogênicas e poligenéticas que definem um fenótipo clínico de apresentação. Dos fenótipos clínicos de defeitos monogênicos, o grande destaque é a Hipercolesterolemia Familiar (HF). Objetivo: realizar uma revisão sistemática da literatura correlacionando a hipercolesterolemia como fator de risco importante para coronariopatias, enfatizando o papel da hipercolesterolemia familiar na doença cardiovascular precoce. Resultados: a literatura analisada demonstra que a LDL-c está estreitamente ligada às doenças cardiovasculares, pois, é a principal lipoproteína envolvida na aterosclerose, e as reduções dos seus níveis refletem diretamente na morbimortalidade desta população. Conclusão: A revisão demonstra a forte relação do aumento da LDL-c no Infarto do miocárdio.
