RESUMOOBJETIVO: Demonstrar o adequado manejo da hipoglicemia e a importância do reconhecimento precoce dessa condição em situações de emergência. METODOLOGIA: Trata-se de um estudo descritivo, do tipo revisão integrativa qualitativa de literatura. Foram coletados artigos usando descritores de pesquisa obtidos na plataforma DeCS, buscando informações sobre hipoglicemia em serviços de emergência e unidades de terapia intensiva. Foram realizadas buscas em três bases de dados: LILACS, SciELO e PubMed. Após a aplicação de critérios de elegibilidade e exclusão, foram selecionados nove estudos para análise crítica. RESULTADOS E DISCUSSÃO: A hipoglicemia é caracterizada por uma queda nos níveis de glicose sanguínea, sendo definida como um ≤ 70 mg/dL. A International Hypoglycaemia Study Group publicou um consenso em 2021 sobre as definições e o manejo da hipoglicemia, que foram amplamente adotados em outras diretrizes. Atualmente, a hipoglicemia é dividida em três estágios, sendo o terceiro caracterizado como hipoglicemia severa, que requer assistência de terceiros para manejo e recuperação. O tratamento da hipoglicemia depende fundamentalmente do nível de consciência do paciente. Quando o paciente está consciente, a via oral é a prioridade, com oferta de carboidratos de rápida absorção assim que observado níveis de glicose sanguínea inferiores a 70 mg/dL. Em um paciente inconsciente, o tratamento preferencial dependerá do local onde o paciente se encontra. Em ambiente extra-hospitalar o glucagon é a primeira linha de tratamento, já em ambiente intra-hospitalar a preferência recai sobre a obtenção precoce do acesso venoso e administração de soluções contendo glicose. CONSIDERAÇÕES FINAIS: Admissão de pacientes inconscientes na emergência é problemática, de tal modo que a hipoglicemia sempre deve ser analisada como hipótese para o quadro. Nesse cenário, terapias emergentes como soluções semelhantes ao glucagon, são promissoras, mas ainda caras, portanto, ainda permanece como primeira escolha no cenário da emergência administrações de soluções contendo glicose.
RESUMOOBJETIVO: avaliar o número de óbitos ocorridos em decorrência da pancreatite aguda e investigar possíveis fatores que possam ter contribuído para o aumento ou redução desses números entre 2006 e 2020 no Brasil. METODOLOGIA: Trata-se de um estudo transversal, descritivo e quantitativo. Os dados foram obtidos através do departamento de informática (DATASUS), sendo os mesmos analisados conforme divisão regional atual da federação: regiões Norte, Nordeste, Sudeste, Sul e Centro-Oeste. Os dados foram posteriormente importados para o software Quantum Gis (QGIS) para elaboração de malhas cartográficas. RESULTADOS E DISCUSSÃO:A análise cartográfica permite inferir que no primeiro período de análise (2006)(2007)(2008)(2009)(2010) o Brasil apresentou um registro de 14.515 óbitos, sendo o Sudeste com o maior registro de óbitos, com 7.575 casos e o Norte com 712 casos. No segundo e terceiro períodos analisados, que apresentam dados entre 2011-2015 e 2016-2020, observou-se aumento significativo no número de óbitos em todas as regiões do país, mantendo a região Sudeste e a Norte com o maior e menor registro de óbitos, respectivamente. Em relação à faixa etária, a pancreatite mostrou-se prevalente em indivíduos com idade superior a 50 anos, com os óbitos correspondendo a 60% do total nessa faixa etária, durante toda a análise. Os óbitos acometeram mais frequentemente o sexo masculino (igual ou superior a 60%) em todos intervalos temporais. CONSIDERAÇÕES FINAIS: Os dados analisados são compatíveis com os resultados recentes da literatura, ainda mantendo o maior número de óbitos em indivíduos do sexo masculino com idade superior a 50 anos e mais concentrados nas regiões Sudeste e Nordeste. Em suma, é importante conscientizar a população sobre a pancreatite aguda e incentivar a adoção de hábitos saudáveis para prevenir essa patologia e reduzir o número de óbitos relacionados a condição no país.
