Objetivo: Avaliar as alterações clínicas e as complicações nos enxertos e nos pacientes com mais de dez anos de transplante renal. Métodos: Estudou-se, retrospectivamente, a evolução de 67 receptores vivos, com enxerto funcionante, submetidos ao primeiro transplante renal, avaliando-se o sexo, a cor, a idade média, a doença de base, a imunossupressão, o tempo médio de transplante e suas complicações e a função renal atual dos receptores. Os doadores foram também avaliados quanto ao sexo, à cor, à idade média e ao tipo e à compatibilidade HLA. Resultados: O tempo médio dos transplantes foi de 13,8 + 2,9 anos e a função renal atual dos pts de 48,7+ 20,7 ml/min. A média de creatinina plasmática aos seis meses e em um ano foi, respectivamente, de 1,50 mg/dl e 1,47 mg/dl. As principais complicações observadas foram infecções, hipertensão arterial, rejeição aguda, nefropatia crônica do enxerto e dislipidemia. Conclusão: A evolução e as complicações observadas em nossos pacientes foram semelhantes às das publicações internacionais, no período anterior ao aparecimento das novas drogas imunossupressoras.
