IntroductIonLe cancer colorectal représente en Belgique environ 6.000 nouveaux cas par an; il s'agit du cancer digestif le plus fréquent pour les deux sexes confondus.Il arrive en troisième position chez l'homme après le poumon et la prostate et en deuxième position chez la femme après le cancer du sein.Il reste associé à une mortalité élevée (40 à 50 % des cas à 5 ans tous stades confondus); en Belgique, on déplore 3.000 décès annuels. L'incidence est faible avant 50 ans, mais augmente ensuite de façon importante avec chaque décade, avec une discrète prédominance chez l'homme. L'incidence du cancer colorectal est en augmentation depuis une dizaine d'années dans les pays du Nord de l'Europe. L'âge moyen au diagnostic est de 72 ans chez la femme et 69 ans chez l'homme (1).Ces caractéristiques épidémiologiques et les nouvelles possibilités thérapeutiques adjuvantes à la chirurgie justifient la mise en place d'un dépistage cohérent et efficace dans les pays à forte incidence comme la Belgique. Dépistage De masseL'objectif d'un dépistage de masse dans le cas présent est de réduire la mortalité du cancer colorectal par un diagnostic plus précoce et par la détection et l'exérèse des adénomes, lésions pré-cancéreuses les plus fréquentes. La population cible est la population à risque moyen, asymptomatique, appartenant à la tranche d'âge 50-75 ans indépendamment de tout antécédent personnel de maladies inflammatoires et de tout antécédent personnel ou familial d'adénomes et/ ou de cancers colorectaux.Les caractéristiques théoriques requises pour un dépistage de masse sont les suivantes : sensibilité satisfaisante, spécificité très élevée, simplicité de réalisation, bonne reproductibilité, risques peu élevés, acceptabilité par le sujet et coût raisonnable (1). Le coût à prendre en compte est celui des procédures de dépistage et de diagnostic, mais aussi le coût engendré par les complications éventuelles des procédures ainsi que le coût de la prise en charge des cancers colorectaux diagnostiqués (1).Au-delà de 75 ans, le bénéfice d'un dépistage par rapport aux risques encourus dépend surtout de l'espérance de vie de l'individu et de son état général (2). L'utilité d'un examen de dépistage doit donc être décidée au cas par cas et les sujets de cet âge ne peuvent être inclus dans un programme pré-établi de dépistage de masse. Le cancer colorectal est un cancer fréquent dans nos pays du Nord de l'Europe ; la mortalité reste élevée malgré de nombreuses et nouvelles possibilités thérapeutiques.Le dépistage de masse, dont le but est de réduire la mortalité de ce cancer, devrait être une priorité politique à l'échelon national.Les techniques de dépistage sont nombreuses, plus ou moins invasives mais aucune ne fait l'objet de recommandations unanimes.A l'avenir, les progrès de la technologie médicale dans les domaines de l'imagerie, de l'endoscopie et de la biologie moléculaire devraient permettre un dépistage plus efficace tout en restant tolérable par le patient ainsi que sur le plan du coût.
