Introduction: SARS-CoV-2 infection produces laboratory abnormalities, which, although they are nonspecific, their values can be useful as prognostic markers. Materials and Methods: Observational, analytical study of cases and controls, with a non-probabilistic sampling of consecutive cases from January to July 2021 in patients with SARS-CoV-2 infection at Hospital National. Results: A total of 237 patients (79 cases and 158 controls) were included, the age range was between 21 and 90 years (53.67±15.11). 66.7% were male. Statistically significant differences were found, with a tendency for deceased patients to present higher values of leukocytes (12,600 vs 9,672; p=0.001), CRP (15.40 vs 10.97; p=0.002), D-dimer (15,725 vs 3,714 ; p=0.004), Ferritin (1312 vs 1049; p=0.020), Troponin I (1.71 vs 1.87; p=0.003). Conclusion: The values of leukocytes, PCR, Dimer D, Ferritin, Troponin I, are significantly associated with mortality in this series of cases.
La acidosis metabólica láctica es un trastorno severo del metabolismo intermediario, que conlleva una elevada tasa de mortalidad. Puede presentarse en el contexto de isquemia tisular extensa, fallo cardiocirculatorio, shock séptico, o desencadenarse por determinados tóxicos (metanol, etanol, etc) y fármacos (metformina y otros). El etanol es un depresor del sistema nervioso central que provoca un cuadro clínico caracterizado por euforia, alteraciones de la conducta, pérdida de la inhibición, ataxia, verborrea y, finalmente, estupor y coma, dependiendo del grado de intoxicación además pueden presentar cuadros convulsivos. En este reporte se presenta el caso de una paciente joven con intoxicación alcohólica.
Introducción: el índice neutrófilo-linfocito es la relación entre el número absoluto de neutrófilos y linfocitos. Su aumento predice la enfermedad grave por COVID-19, inclusive cuando la inflamación es subclínica. Objetivo: determinar la utilidad del índice neutrófilo linfocito como predictor de gravedad de la neumonía a SARS-CoV-2. Materiales y métodos: se realizó un estudio de casos y controles, con un muestreo no probabilístico de casos consecutivos de enero a julio de 2021 en pacientes con infección a SARS-CoV-2 del Hospital Nacional, Paraguay. Se consideraron casos a los pacientes con neumonía a SARS-COV-2 grave y controles a aquellos con formas leves y moderadas de neumonía. Resultados: se incluyeron 310 pacientes (155 casos y 155 controles). La franja etaria estuvo comprendida entre 19 a 90 años (media 53±15 años). La diferencia de índice neutrófilo-linfocito resultó 11,71 en casos vs 7,09 en controles (p 0,0001), teniendo 5,08 veces más probabilidades de desarrollar una neumonía grave los pacientes con índice neutrófilo-linfocito mayor a 3. Conclusión: el índice neutrófilo-linfocito predice el desarrollo de una neumonía grave a SARS-CoV-2.
