We studied the effect of clonazepam in a double-blind trial on 12 parkinsonian patients with hypokinetic dysarthria. Speech samples were judged on 14 of the dimensions used in the Mayo Clinic dysarthria study. Of the 11 patients who completed the study, 10 showed improvement. The effective dosage of clonazepam was 0.25 to 0.5 mg/d with higher dosage than that less effective. Clonazepam has a definite role in the management of parkinsonian dysarthria.

Neuropsychologists are occasionally asked to have neuropsychological testing observed via the presence of a third party, through one-way mirrors, or with audio or video monitoring or recording devices. The primary reasons for not allowing observation are its effect on the validity of the examination results and the security of copyrighted test materials. To overcome the problem of observer effects on the examinee's performance, some individuals have suggested that examinations be monitored or recorded without the examinee's awareness (i.e., secretly). However, secretive recording of neuropsychological interviews and testing is deceptive, which is inconsistent with ethical principles. In addition, such recording may affect the behavior of the examiner. For these reasons, neuropsychologists do not, and should not, encourage, condone, or engage in secret recording of neuropsychological interviews or testing.

Objective TTTS is a rare condition occurring in approximately 5-15% of monochorionic twins (sharing a single placenta), in which there is an abnormal arterio-venous connection resulting in excess blood flow to one fetus (“recipient”) and too little to the other (“donor”) (Graeve et al., 2012). Consequently, the “donor” develops in low amniotic fluid, while the “recipient” develops in high amniotic fluid. Neurologic and cardiogenic abnormalities have been identified in the neonatal period and in the long-term for both twins (Wagner, Repke, & Ural, 2013). In terms of neurodevelopmental morbidities, 11.6% of children (age 6) endure mild neurologic impairment and 8.9% experience severe neurologic impairment (Graeve et al., 2012). Methods The present case involves a 7-year-old, right-handed, single female with a medical history of TTTS (“donor”) referred for neuropsychological testing to explore challenges with learning, attention, memory, reading, and spelling. Upon initial neuropsychological consultation, there were suspected neurodevelopmental optometric deficits, so the patient was also referred for ocular/vision testing. This revealed a significant convergence insufficiency/binocular dysfunction (eye teaming), ocular motor dysfunction (tracking), and visual disturbance (visual sensory overload) deficiencies. Results Neuropsychological testing revealed a neurodevelopmental disorder characterized by a language learning disability with mixed-dyslexia, ADHD - combined type, an executive functioning disorder, and neuro-optometric deficiencies interfering with neurocognitive functioning. Conclusion The present case provided further evidence of long-term neurodevelopmental sequelae associated with TTTS (“donor”). Neuropsychological testing elucidated the severity and type of neurodevelopmental impairments and was instrumental in the patient receiving an Individualized Education Program (IEP), which incorporated the accommodations contained in the neuropsychological diagnostic report.

By virtue of their extensive knowledge base and specialized training in brain-behavior relationships, neuropsychologists are especially poised to execute a unique broad-based approach to overall cognitive wellness and should be viewed as primary care providers of cognitive health. This article will describe a novel comprehensive cognitive wellness service delivery model including cognitive health, anti-aging, lifelong wellness, and longevity-oriented practices. These practice areas include brain-based cognitive wellness, emotional and spiritually centric exploration, and related multimodality health interventions. As experts in mind-body connections, neuropsychologists can provide a variety of evidence-based treatment options, empowering patients with a sense of value and purpose. Multiple areas of clinical therapy skill-based learning, tailor-made to fit individual needs, will be discussed including: brain stimulating activities, restorative techniques, automatic negative thoughts and maladaptive thinking reduction, inflammation and pain management techniques, nutrition and culinary focused cognitive wellness, spirituality based practices and mindfulness, movement and exercise, alternative/complimentary therapies, relationship restoration/social engagement, and trauma healing/meaning. Cognitive health rests upon the foundation of counteracting mind-body connection disruptions from multiple etiologies including inflammation, chronic stress, metabolic issues, cardiac conditions, autoimmune disease, neurological disorders, infectious diseases, and allergy spectrum disorders. Superimposed on these issues are lifestyle patterns and negative health behaviors that develop as ill-fated compensatory mechanisms used to cope with life stressors and aging. The brain and body are electrical systems that can "short circuit." The therapy practices inherent in the proposed cognitive wellness service delivery model can provide preventative insulation and circuit breaking against the shock of illness.
