RESUMEN En Cuba, el acceso y la cobertura universales de salud descansan sobre tres principios clave: la salud como derecho humano, la equidad y la solidaridad. Aunque muchos de los indicadores de salud cubanos están entre los mejores de la Región de las Américas, en el 2011 se decidió reorganizar los servicios de salud, a tono con el proceso de actualización del modelo económico y social cubano que transcurría en todos los sectores del país. Para ello, se diseñó un proyecto de investigación-acción que abarcó el diagnóstico de la situación, la implementación de los cambios y la evaluación de los resultados, en varias etapas. Como resultado, se racionalizaron los recursos humanos con una reducción de más de 150 000 plazas no vinculadas directamente a la atención del paciente, se aligeraron las estructuras de dirección en 57 municipios, se compactaron 46 policlínicos, se optimizó el Programa del Médico y la Enfermera de la Familia con la proyección comunitaria de 20 especialidades, se reorganizó la docencia y se reordenaron los programas de cooperación médica internacional. Estos cambios han contribuido a mejorar la sostenibilidad del Sistema Nacional de Salud y su desempeño: aumento en el número de consultas en el nivel primario (19,3%) y estomatológicas (56,6%), reducción del número de consultas en cuerpo de guardia (16,1%), aumento del número de pacientes intervenidos quirúrgicamente (12,1%), incremento del número de proyectos de investigación (300%) y crecimiento en el número de estudiantes de Medicina (55,7%), entre otros. El proyecto de transformaciones en la salud emprendido en Cuba continúa.
RESUMENLa Salud Pública Cubana muestra un empeño manifiesto por la salud de la población, de ahí la conveniencia de exponer consideraciones sobre el proceso de inserción en ella de la intersectorialidad como única opción posible para su desarrollo. Hacer referencia a la salud como resultante de un proceso complejo que se construye entre todos, justifica las acciones intersectoriales como recurso imprescindible en la preservación y en la solución de los problemas de salud de la población. Se concluye que la intersectorialidad ha estado presente en las políticas del Estado Revolucionario, desde su interpretación empírica y filosófica hasta la tecnológica y que esta tecnología salubrista, al facilitar la intervención coordinada de los sectores de la economía y la sociedad con el sector de la salud, ha posibilitado una dirección más eficaz y eficiente de todos los niveles del sistema que se manifiesta en indicadores de salud pública relevantes.Palabras clave: Salud Pública Cubana, intersectorialidad, voluntad política, determinantes de la salud, salud de la población, problemas de salud. 2 ABSTRACTThe Cuban public health care shows its clear determination to protect the population's health, so it is advisable to state some considerations on the insertion of intersectoriality in this process as the only possible option for public health development. Making reference to health as the result of a complex process that is built by all of us justifies the implementation of intersectoral actions as an indispensable tool for health preservation and resolution of health problems. It was concluded that intersectoriality has been present in the revolutionary state's policies, from its empirical and philosophical to technological interpretation, and that this health technology has facilitated the coordinated involvement of the different economic and social sectors in the health care sector and in this way, it has allowed more efficient and effective management at all levels of the system that materializes in relevant public health indicators.

RESUMENEl sistema de salud tiene como objetivo incrementar los niveles de salud, la calidad de los servicios, y la satisfacción de la población y de los prestadores. Para alcanzar estos propósitos, le concierne relacionarse, establecer alianzas e integrarse de manera sinérgica con otros sistemas de la sociedad y la economía a fin de ejercer control sobre los determinantes sociales de la salud. El propósito de este trabajo es identificar la percepción de actores sociales acerca del proceso de la intersectorialidad en su sector y sus implicaciones en la salud y el bienestar de la población. Se obtuvo información de 52 actores sociales clave en el contexto socioeconómico actual. Se pudo apreciar que a pesar de existir voluntad política y actores sociales convencidos, la realización del proceso de intersectorialidad para la salud y el bienestar en el contexto socioeconómico cubano, se percibe como baja, y que muchas de las situaciones que demandan intersectorialidad en la vida diaria demuestran insuficiente integración sectorial, ello indica que su aplicación en la actualidad no se corresponde con el alcance tecnológico que la intersectorialidad ha adquirido, por lo que hay necesidad de reforzar los elementos que competen al sector de la salud, y los vinculados a otros sectores de la sociedad y la economía.Palabras clave: intersectorialidad, contexto social, salud.
