Résumé L’acceptation de plus en plus fréquente de la notion de handicap psychique s’est accompagnée d’un besoin de délimitation précise de ses contours et de dénombrement de la population concernée. La difficulté d’estimation de la prévalence du handicap en général tient notamment à son caractère multidimensionnel et au fait que chacune des dimensions relève d’un continuum. Dans le domaine des troubles mentaux, le contexte historique de l’émergence de la notion de handicap psychique a probablement contribué à la proximité de cette entité avec celle de maladie mentale. L’appréciation d’une prévalence s’appuie nécessairement sur des outils conceptuels visant à cerner le phénomène. L’étude des classifications des maladies (CIM), des troubles mentaux (DSM) et du handicap (CIH et CIF) ainsi que des questionnaires d’enquête basés sur ces classifications attestent du recouvrement partiel de ces outils, ce qui complique l’obtention d’une quantification univoque du handicap psychique en population générale.
SES disparities in the older age group were significant and large in absolute terms when considering more severe outcomes such as stroke dependency. These findings question the ability of universal health care systems to answer equitably the need of the aging population.
Résumé L’objet de ce texte est de proposer une synthèse des travaux réalisés, à partir de l’enquête HID, sur un ensemble de domaines qui relèvent du thème général de la vie dans la cité : logement, transports, scolarité, culture, loisirs, sports, vie sociale et relations sociales et sexuelles. Cette synthèse qui concerne la vie à domicile et certains aspects de la vie en institution porte principalement sur les personnes de moins de 60 ans. Les travaux relatifs aux différents domaines tendent à mettre en évidence l’impact négatif des déficiences ou incapacités ainsi que celui du statut social défavorisé sur les possibilités de participation à la vie dans la cité. Ils développent aussi des approches nouvelles portant sur le poids de l’environnement et de ses obstacles architecturaux et techniques. Les difficultés liées à ces obstacles font assez souvent l’objet d’une estimation quantitative, quasi inexistante jusqu’ici. On observe aussi que les répondants imputent beaucoup moins fréquemment leurs difficultés aux obstacles environnementaux qu’à leur état de santé. Ce résultat laisse penser que des progrès pourraient être faits dans la diffusion de la connaissance de l’accessibilité, des aménagements et de la technologie.
This article deals with the development of a basis for data collection and statistics on disability in France. After having recalled the first attempts to establish statistics on disability, highlighting the lack of a reliable conceptual basis, its shows how ICIDH spread out from the geriatrics research field to the disability research field, followed by public management of disability and health care, and recently by the national statistics administration. The article aims at showing that the use of ICIDH always needed adaptations to fit in the forms of data collection and questionnaires. It also demonstrates that its tri-dimensional conception of disability considerably helped to specify the kind of data that were expected and the way they could be organised to enrich the view on disability. In its last part, it mentions what the French reactions were to the submission of ICF by WHO at the World Health Assembly in May 2001 and what could be the consequences in the matter of statistics on disability in France.
