Σκοπός: Στελέχη K. pneumoniae που παράγουν μεταλλο-β-λακταμάσες (MBL) τύπουVIM έχουν προσφάτως περιγραφεί ως αίτια απειλητικών για τη ζωή λοιμώξεων. Σκοπόςτης παρούσας μελέτης ήταν ο προσδιορισμός των παραγόντων κινδύνου γιαβακτηριαιμία από VIM-θετικά στελέχη και η αξιολόγηση της κλινικής σημασίας τηςπαραγωγής MBL τύπου VIM.Υλικό-μέθοδος: Πραγματοποιήθηκε προοπτική μελέτη σε 3 νοσοκομεία της Αθήνας από2004 έως 2006. Ασθενείς με βακτηριαιμία από K. pneumoniae εντοπίσθηκαν καιπαρακολουθήθηκαν μέχρι το τέλος της νοσηλείας τους και οι κλινικές πληροφορίεςκατεγράφησαν σε προσχεδιασμένο ερωτηματολόγιο. Η ευαισθησία στα αντιβιοτικάπροσδιορίστηκε με Etest. Εξετάσθηκαν η παραγωγή μεταλλο-β-λακταμασών και ευρέοςφάσματος β-λακταμασών με τη δοκιμασία διάχυσης των δίσκων, η παρουσία τουυπεύθυνου γονιδίου blaVIM-1 με PCR και η κλωνικότητα των στελεχών με PFGE.Αποτελέσματα: Συμπεριελήφθησαν 178 ασθενείς με βακτηριαιμία από K. pneumoniae,67 (37.6%) από VIM-θετικά και 111 (62.4%) από VIM-αρνητικά στελέχη. Οι MIC της ιμιπενέμης για τα VIM-θετικά στελέχη κυμαίνονταν από πλήρη ευαισθησία έως υψηλούεπιπέδου αντοχή: 19 VIM-θετικά στελέχη ήταν ανθεκτικά στην ιμιπενέμη και 48ευαίσθητα, ενώ κανένα από τα VIM-αρνητικά στελέχη δεν παρουσίαζε αντοχή στιςκαρβαπενέμες. Όλα τα VIM-θετικά στελέχη ήταν πολυανθεκτικά. Η προηγούμενη χρήση>3 διαφορετικών κατηγοριών αντιβιοτικών (OR, 12.6; 95% CI, 2.17-73.27; P=0.01), ηπροηγούμενη χρήση καρβαπενέμης (OR, 2.83, 95% CI 1.07-7.49, P 0.03) και η νοσηλείασε Μονάδα Εντατικής Θεραπείας (OR, 6.78, 95% CI 2.69-17.06, P<0.001) ήτανανεξάρτητοι παράγοντες κινδύνου για βακτηριαιμία από VIM-θετικό στέλεχος. Οιασθενείς με λοίμωξη από VIM-θετικά και VIM-αρνητικά στελέχη είχαν συγκρίσιμηθνητότητα. Αντίθετα η αντοχή στις καρβαπενέμες (MICs για την μεροπενέμη και τηνιμιπενέμη > 4 μg/ml) είχε σημαντική επίδραση στη θνητότητα στις 14 ημέρες μετά τηβακτηριαιμία. Επίσης, η πολυμικροβιακή βακτηριαιμία και το Pitt bacteremia score ≥4σχετίζονταν με μεγαλύτερη θνητότητα. Η ηλικία των ασθενών, η βαρύτητα τουυποκείμενου νοσήματος, η εστία της βακτηριαιμίας και η χορήγηση ακατάλληλης εμπειρικής ή οριστικής θεραπείας παρουσίαζαν επίσης τάση για μεγαλύτερη θνητότητα.Με την εισαγωγή στο μοντέλο Cox regression όλων των σχετιζόμενων με τον ξενιστήκαι τη λοίμωξη μεταβλητών, των οποίων η τιμή του p ήταν ≤ 0.10 στηνμονοπαραγοντική ανάλυση, η ΜΙC στις καρβαπενέμες > 4 μg/ml (HR, 2.83; 95% CI,1.08-7.41; P= 0.035), η ηλικία (HR, 1.03; 95% CI, 1.01-1.06; P= 0.021) και το ταχέωςθανατηφόρο υποκείμενο νόσημα (HR, 2.84; 95% CI, 1.26-6.39; P= 0.012) σχετίζοντανμε τη θνητότητα στις 14 ημέρες. Μετά την ενσωμάτωση της μεταβλητής “ακατάλληληεμπειρική θεραπεία” ή “ακατάλληλη οριστική θεραπεία,” η επίδραση της αντοχής στιςκαρβαπενέμες στη θνητότητα στις 14 ημέρες μειώθηκε σε επίπεδο μη στατιστικήςσημαντικότητας ενώ οι τιμές των HR και p των άλλων μεταβλητών παρέμεινανουσιαστικά ίδιες. Χαμηλότερα ποσοστά θνητότητας (8.3%), είχε η ομάδα των ασθενώνπου έλαβε συνδυασμένη θεραπεία με δύο δραστικά φάρμακα, εκ των οποίων το ένα ήτανκαρβαπενέμη και το άλλο κολιστίνη ή μια δραστική αμινογλυκοσίδη. Αντίθετα, οιασθενείς που έλαβαν θεραπεία με ένα δραστικό φάρμακο (καρβαπενέμη, κολιστίνη ή μιααμινογλυκοσίδη) είχαν ποσοστά θνητότητας παρόμοια με αυτά των ασθενών που έλαβανακατάλληλη θεραπεία (κανένα δραστικό φάρμακο).Συμπεράσματα: Τα στελέχη K. pneumoniae που παράγουν μεταλλο-β-λακταμάσεςτύπου VIM αποτελούν σημαντικά νοσοκομειακά παθογόνα στη χώρα μας και προκαλούνθανατηφόρες λοιμώξεις, κυρίως σε ασθενείς των Μονάδων Εντατικής Θεραπείας και σεασθενείς με προηγούμενη έκθεση στις καρβαπενέμες. Η βακτηριαιμία από VIM-θετικάστελέχη χαρακτηρίζεται από αυξημένη θνητότητα, όταν το στέλεχος είναι ανθεκτικό στιςκαρβαπενέμες.
