Resumo Objetivo: verificar as condições de saúde bucal de trabalhadores de cinco indústrias da região metropolitana de Salvador, Bahia, que participavam do Programa de Saúde Bucal na Empresa (PSBE) entre os anos 2003 a 2008. Métodos: estudo transversal retrospectivo, com dados secundários do banco de dados do SESI/DR-BA. Participaram todos os funcionários que realizaram pelo menos dois exames odontológicos no período, totalizando 1.277 trabalhadores. Foram investigados dados sociodemográficos, incidência de cárie, atividade de cárie, urgência odontológica, uso e necessidade de prótese. Resultado: predominou o sexo masculino com média de idade de 38 anos (DP = 10,07). O índice CPO-D (dentes cariados, perdidos e obturados) apresentou variação da média de 15 a 19, com maior percentual para dentes obturados (de 58,5% a 70,2%). Observou-se menor incidência de cárie nos que possuíam maior número de exames – 35,03% com 2 exames e 21,88% com 4 exames (p<0,01). Destaca-se maior associação positiva entre atividade de cárie e ramo de metais (29,19%) em 2007. O ramo outros serviços apresentou os maiores percentuais de urgência odontológica (5,60% em 2007, 8,40% em 2008). Conclusão: a redução na incidência de cáries e na necessidade de próteses entre os que mais realizaram exames indica que programas de saúde bucal nas empresas podem contribuir para a melhor saúde bucal dos trabalhadores.
The aim of this research was to identify studies describing the educational and/or health-promoting interventions and strategies used in various programs of oral healthcare conducted in populations of industrial workers. A review of the Science, MEDLINE, LILACS and SCIELO databases was performed for the period between 1980 and 2009. The criteria for a publication to be in- cluded were: 1) the population had to consist of adult workers; and 2) the educational and/or health-promoting interventions had to have been described. A great variety of educational strategies was found, ranging from individual counseling to small-group discussions and instructions on oral hygiene with supervised tooth brushing. The presence of plaque and gingivitis were the most commonly used indicators of the effect of these interventions. Cost evaluation was also an important topic. In general, the indicators analyzed reflected positive results that included a reduction in the indicators themselves (plaque and gingivitis) and in the cost of the programs, suggesting a certain effect of the educational programs developed. Nevertheless, differences in forms of exposure according to the type of occupation are not discussed and neither are specific social determinants such as income and education level. The quality of the studies is limited, which reduces the robustness of the evidence on the effect of educational programs on the work population. Studies should be carried out using mixed approaches to evaluate the efficacy of these programs and propose improvements
RESUMOO líquen plano (LPO) é uma doença inflamatória crônica que acomete pele e mucosa e atinge em média 1 a 2% da população, sendo mais comum em mulheres de meia idade. Apesar da etiologia desconhecida, muitos achados sugerem que essa doença é mediada por reações imunológicas. O presente estudo consistiu em uma análise da distribuição das células mastocitárias, através do azul de toluidina. A população mastocitária foi contada por dois observadores, na área correspondente à inflamação liquenoide e na área imediatamente após, e comparada com a de um fragmento de mucosa bucal normal. Os resultados mostraram um número médio maior de células mastocitárias nas áreas correspondentes à inflamação liquenoide, em comparação com o da mucosa oral normal. Isto foi comprovado estatisticamente através do teste de Kruskal-Wallis, no qual houve diferença significativa (p < 0,005) entre as médias de mastócitos encontradas. Do estudo, concluiu-se que existe uma importante interação entre a população de células mastocitárias e os linfócitos presentes na lesão, os quais exercem um papel marcante na patogênese e cronicidade do líquen plano oral. Palavras-chave: Líquen Plano Bucal. Mastócitos. Abstract:Lichen planus is a chronic inflammatory disease affecting skin and mucosae. It may reach 1-2% of the population, being more common in middle-aged women. Despite the unknown etiology, many findings suggest that this disease is mediated by immunological reactions. The aim of this was to analyse the distribution of mast cells by toluidine blue. The mast cell population was counted by two observers in the area corresponding to the lichenoid inflammation, and the area immediately after, and compared to normal oral mucosa. The results showed a larger average number of mast cells in areas corresponding to the lichenoid inflammation compared to normal oral mucosa. This was confirmed statistically by the Kruskal-Wallis, as there was significant difference (p<0,005) between the mast cell mean found. In conclusion, there is an important interaction between the population of mast cells and lymphocytes present in the lesion, which exerts a pivotal role in the pathogenesis and chronicity of oral lichen planus.
