A videomediastinoscopia (VM) é uma técnica cirúrgica que possibilita o estadia¬mento e o diagnóstico do câncer de pulmão. Objetivo: Avaliar a eficácia da VM em com¬paração à mediastinoscopia convencional (MC) realizadas com a finalidade de esta¬diar e/ou diagnosticar câncer de pulmão em pacientes atendidos no Serviço de Cirurgia Torácica do Hospital Universitário Gaffrée e Guinle (SCT-HUGG). Métodos: Quarenta pacientes submetidos à VM entre os anos 2012-2015 foram agrupados e analisados segundo: sexo, idade, estações linfonodais biopsiadas, evidência na Tomografia Com¬putadorizada (TC) de tórax de linfonodos mediastinais e massa ou nódulo pulmonar associado à linfonodomegalia, realização prévia de broncoscopia, resultados histopa¬tológicos e cirurgia complementar. Quarenta e três procedimentos de MC realizados entre os anos 1994-2012 também foram analisa¬dos. Resultados: Na VM, a média de idade dos pacientes foi 50,5 anos; 19 (58,6%) com histórico de tabagismo. Houve um total de 86 biópsias em linfonodos mediastinais dis¬tribuídos nas estações 2R, 2L, 4R, 4L e 7, média de 2,28 biópsias por paciente. Foram diagnosticados 10 casos (31%) de câncer de pulmão com acometimento linfonodal. Não houve complicações intra e pós-operatórias relacionadas ao procedimento. Na MC, ob¬teve-se um total de 58 biópsias e foram diag¬nosticados 7 casos (16,27%) de câncer de pulmão nos linfonodos mediastinais. Con¬clusão: A VM mostrou-se importante para o estadiamento do câncer de pulmão por ser um método minimamente invasivo, seguro, eficaz e com especificidade diagnóstica de 100%.
