A median safinamide (SAF) dose of 70 mg/day (range 40 to 90 mg/day) increased the percentage of parkinsonian patients improving their motor scores by > or =30% from baseline (responders) after 3 months from 21.4% (placebo) to 37.5% (p < 0.05, calculated by logistic regression analysis). In a subgroup of 101 patients under stable treatment with a single dopamine agonist, addition of SAF magnified the response (47.1% responders, mean 4.7-point motor score decrease; p > or = 0.05). These results suggest that doses of SAF exerting ion channel block and glutamate release inhibition add to its symptomatic effect and warrant exploration of higher doses.
The anion gap was evaluated in 26 consecutive patients with multiple myeloma (15 IgG and 11 IgA). The average anion gap was found reduced only in IgG myeloma. Instead, in the subjects with IgA myeloma it resulted as increased. The partial differences between our results and those of other authors are discussed. The finding of correlations between the anion gaps and paraproteinemic concentrations (direct in IgA myeloma, inverse in IgG myeloma) gives further support to the conception that the charge exhibited in the serum by IgG and IgA paraproteins is, respectively, positive and negative. It is suggested that a ‘normalization’ of the anion gap during chemotherapy may be a useful tool to judge the efficacy of the treatment of multiple myeloma.
Post mastectomy reconstruction is nowadays an integral part of breast cancer treatment, usually performed in the immediate setting. Among women with hereditary gastric and breast cancer syndromes, three different scenarios can be identified. First, healthy women seeking for prophylactic mastectomies. Second, cancer patients requiring mastectomy at the tumor site and simultaneous risk reducing mastectomy of the healthy breast. Third, cancer patients who have been treated for primary cancer requiring risk reducing mastectomies in a further stage. In this paper, we present a schematic guide for reconstruction for each subpopulation of subjects and their peculiarities.
A case of isolated λ-light chain proteinemia and proteinuria is reported. During a 42-month follow-up, no sign of myelomatosis or amyloidosis developed and Bence Jones proteinuria remained nearly of the same magnitude. This case of apparently idiopathic Bence Jones gammopathy, the first of λ-type so far reported, is added to similar observations to give further support to the contention that a benign counterparty to light-chain myeloma may exist.
