Both metformin and MYO, can be considered as first line treatment for restoring normal menstrual cycles in most patients with PCOS, even if MYO treatment seems to be more effective than metformin.

To evaluate retrospectively the prevalence of gestational diabetes (GD) in pregnancies obtained with myo-inositol administration in women with polycystic ovary syndrome. A total of 98 pregnancies in PCOS women obtained in a 3-year period, either with myo-inositol (n. 54), or with metformin (n. 44) were considered. While myo-inositol was assumed through the whole pregnancy, the group of women treated with metformin stopped the drug assumption after pregnancy diagnosis, and was considered as a control group. After having eliminated cases of miscarriages and twin pregnancies, a definitive number of 46 women in the myo-inositol group and 37 in the control group was taken in account to be retrospectively evaluated. The primary outcome measure was GD occurrence in both groups; whereas secondary outcome measures were pregnancy outcomes: hypertensive disorders, pre-term birth, macrosomia and caesarean section occurrence. Prevalence of GD in the myo-inositol group was 17.4% versus 54% in the control group, with a highly significant difference also after adjusting for covariates. Consequently, in the control group the risk of GD occurrence was more than double compared to the myo-inositol group, with an odds ratio 2.4 (confidence interval 95%, 1.3-4.4). There was no difference between the groups in relation to secondary outcome measures. This study suggests a possible effect of myo-inositol in the primary prevention of GD in PCOS women.

The gradient of environmental conditions from west to east in the Mediterranean results in very low primary productivity in the eastern area of this sea. This impoverishment is expressed also in higher trophic levels and has been accounted for by several faunistic phenomena. One of these is 'Levantine nanism' (dwarfism); this is characterized by smaller body size of specimens in the Levantine basin compared with conspecifics in the western Mediterranean. Nanism has been hypothesized for various taxonomic groups in the Mediterranean, but no quantitative study has yet been carried out to confirm it. In the present study male and female red mullet Mullus barbatus from trawl surveys carried out along the Mediterranean coast of Israel and the Strait of Sicily were sampled. Each fish was sexed, measured for total length (TL) and aged by otolith readings. ANCOVA analyses indicated that the TL of both males and females from Israel was significantly smaller at increasing ages than conspecifics of the same age and sex from Sicily. In addition, preliminary examination of sexual maturity of M. barbatus of both sexes indicated that the Israeli fish sexually mature at a smaller size than conspecifics of the same sex from Sicily. These findings can be explained by low productivity in the Levantine basin compared with the western Mediterranean. The low and unpredictable food supply in the southeastern Mediterranean may result in a form of r-strategy of the marginal eastern populations of this species that leads to early reproduction and smaller body size. The average higher water temperature may also partly explain Levantine nanism, as it may cause more intensive metabolic processes in the southeastern population, resulting in earlier sexual maturity and cessation of growth.

Changes in spawning-stock structure and recruitment pattern of red mullet, Mullus barbatus, after a trawl ban in the Gulf of Castellammare (central Mediterranean Sea). – ICES Journal of Marine Science, 65: 1175–1183. The increase in biomass of red mullet, Mullus barbatus, in the Gulf of Castellammare (northwestern Sicily, central Mediterranean) after a 14-year trawl ban, prompted us to compare the spawning-stock structure and the recruitment pattern before and after the closure. Datasets obtained from three experimental trawl surveys were available before the ban (April and September 1985; April–May 1986) and four post-ban (September and November 2004; March and May 2005). Spawning-stock biomass increased significantly after the ban. Moreover, females at depths >50 m in the post-ban period were larger than those collected before the ban at the same depth. The recruitment pattern of the population also changed. Notably, recruit numbers increased and recruitment occurs over a broader period. The increase in biomass after the trawl ban seems to be the result of a combination of different processes, mainly associated with the lowering of fishing mortality. A positive trend in sea surface temperature in the area may have played a role too.
