Background. A number of studies have been aimed at solving the problems of introducing information technology and systems, but the questions of informatization in rural medicine are not completely resolved. It is important to optimize the prognosis of diseases using available and inexpensive information methods for the improvement of primary healthcare. Objectives. The aim of our study was to develop an algorithm to optimize the decision-making prognosis of disease at the primary health care level based on information methods. Material and methods. The data used for analysis originated from the survey results of 63 patients with hypertension in educational and practical centers of primary health care (EPCPHC) of Ternopil region (Ukraine). For a deeper analysis and clustering, the neural network approach was used with the NeuroXL Classifier add-in application for Microsoft Excel. Results. Thirteen (19.40%) patients experienced health deterioration and the development of complications. It has been established that neural network clustering could effectively and objectively allocate patients to the appropriate category in terms of the average survey results. Cluster analysis results have shown that the combination of high blood pressure (systolic, diastolic and pulse) gave reason to anticipate the deterioration of patients' conditions. Conclusions. A decision algorithm was created in order to optimize the prediction of diseases at the primary health care level, and also to correct examination and treatment based on an analysis of average values of patients' examination and the use of neural network clustering.
Objective. To propose the prognostication method for the complications occurrence after operative interventions, basing on application of multiparametric neuro-web clasterisation with subsequent elaboration of scale for their development risk stratification. Materials and methods. The indices of examination were analyzed in 411 patients, suffering obliterating atherosclerosis of main arteries of the lower extremities. The neuro-web clasterization, using a software raising NeuroXL Classifier, was conducted for more deep analysis of concomitant changes in the indices investigated for the risk prognostication optimization of the morbidity development in postoperative period. Results. The scale of the risk stratification for development of postoperative complications after interventions on the main arteries of the lower extremities proposed, takes into account multifactor character of clinic-anamnestic and laboratory-instrumental investigations. Taking into account the concomitant factors presence, which characterize the organs and systems state, they impact the choice of method of reconstructive operative intervention. Conclusion. Basing on data of the neuro-web clasterization, it is possible to prognosticate the complcations after operative intervention on the main arteries of the lower extremities. There were delineated four levels of the complications development risk in postoperative period: very high – 31-40 points, high – 21-30 points, moderate – 11-20 points, and low – 1-10 points.
Ефективне використання інформаційних технологій дає можливість забезпечити порівняльний аналіз багатьох чинників у наукових медичних дослідженнях, що особливо важливо у морфології, де необхідно здійснювати аналіз значної за обсягом цифрової інформації. Особливо актуальним являється розроблення нових методик комплексного порівняльного аналізу морфологічних змін тканин і біохімічних порушень у результаті розвитку турнікетного синдрому, що є одним із найпоширеніших видів пошкоджень у військовій і цивільній медицині. У роботі запропоновано методику оптимізації прогнозування тяжкості морфологічних порушень при експериментальній гострій ішемії-реперфузії на основі поєднаних змін біохімічних показників і показників проксидантно-антиоксидантної системи з використанням нейромережевої кластеризації. Експериментальна модель ішемічно-реперфузійного ураження представлена п'ятьма групами щурів із термінами реперфузії 1 і 2 години, 1 доба, 7 та 14 діб (по 6 тварин у кожній). Контрольна група нараховувала 6 тварин. Підхід ґрунтується на застосуванні нейромережевої кластеризації із використанням надбудови NeuroXL Classifier для програми Microsoft Excel. Із метою більш глибокого аналізу та встановлення значення поєднання змін тих чи інших параметрів здійснено нейромережеву кластеризацію біохімічних показників, показників ПОЛ та АОЗ сироватки крові білих щурів у різні періоди після зняття турнікету. Встановлено найбільше прогностичне значення тяжкості морфологічних порушень у ранньому реперфузійному періоді за даними нейромережевої кластеризації поєднаних змін рівня креатиніну, холестерину, АЛТ, АсАТ, лужної фосфатази, трієнових кон'югант і ТБК-АП. Отже, для оптимізації прогнозування розвитку морфологічних ішемічних порушень при експериментальній гострій ішемії-реперфузії запропоновано методику аналізу результатів експериментального дослідження на основі змін морфологічних, біохімічних показників, показників продуктів ПОЛ та антиоксидантного захисту із застосуванням нейромережевої кластеризації.
Abstract. The growing number of foreign students and their countries of origin creates new challenges to universities. I. Horbachevsky Ternopil Medical National University is committed to creating and maintaining an educational, working, and living environment free from discrimination and harassment. Initially, the English classes for teachers were organized. Later, the position of deputy dean of the Faculty of Medicine for the education of foreign students was introduced. After the creation of the Dean’s Office of International Students’ Faculty, the number of foreign students increased. At the request of the legislation, the University established the Committee for Prevention and Response to Sexual Harassment at TNMU, the Committee for Prevention and Response to Discrimination at TNMU, and the Committee for Review of Bullying Cases at TNMU. The article also highlights the process of creating of the Center for the Prevention of Discrimination and Harassment and its work at I. Horbachevsky Ternopil National Medical University and describes cases of abuse by students with an attempt to use the center for their own purposes or to take revenge on teachers.
