Введение. Дисплазия соединительной ткани (ДСТ) оказывает влияние на патогенез и течение многих заболеваний, в том числе и пищеварительной системы.Цель исследования -определить характер и выраженность морфофункциональных изменений в слизистой оболочке (СО) желудка у детей с хроническим гастритом (ХГ), развивающимся на фоне разной выраженности ДСТ.Материал и методы. Обследованы 194 ребенка школьного возраста, страдающих ХГ (101 пациент с легкой степенью ДСТ, 57 -с выраженной, и 36 пациентов без нарушения развития соединительной ткани (СТ). У всех пациентов проводилась эзофагогастродуоденоскопия (ЭГДС) с прицельной биопсией СО желудка с последующим их детальным гистологическим, морфометрическим и статистическим анализом. Наличие и тяжесть ДСТ верифицировали по критериям Т. Милковска-Димитровой.Результаты. У пациентов с ХГ на фоне ДСТ чаще встречались деструктивные изменения желудка. По мере увеличения степени тяжести дисплазии установлено уменьшение высоты эпителиального покрова, вплоть до уплощения и десквамации с обнажением собственной пластинки СО. Выявлено снижение защитных свойств СО при сохранении агрессивных факторов желудочного сока, что способствует развитию деструктивных поражений.Заключение. По мере нарастания степени выраженности диспластических изменений в соединительной ткани наблюдается рост частоты деструктивных процессов в слизистой оболочке желудка.Ключевые слова: дети, хронический гастрит, слизистая оболочка, дисплазия соединительной ткани. Background. Connective Tissue Dysplasia (CTD) affects the pathogenesis and clinical course of many diseases, including those of the digestive system.Objective -to determine the nature and severity of morph-functional changes in stomach mucous membrane in children with chronic gastritis associated with different CTD severity. Material and methods. 194 school-age children with chronic gastritis were examined (101 patients with mild CTD, 57 with severe and 36 patients without any connective tissue developmental disorders). Esophagogastroduodenoscopy with a targeted biopsy of the stomach, followed by detailed histological, morphometric and statistical analysis was performed for all patients. The presence and severity of CTD were verified according to the criteria of T. Milkovska-Dimitrova. Results. In patients with chronic CTD-related gastritis destructive changes in the stomach were more common. As the severity of dysplasia increases, there has been noted a decrease of the epithelial layer height up to its flattening and desquamation. We have revealed a decrease of the protective properties of mucosa in the presence of aggressive factors of gastric juice, that contributes to the destructive lesions development.Conclusion. The incidence of destructive processes in the gastric mucosa increases alongside with the rise of the severity of the connective tissue dysplastic changes.
Введение. Бронхиальная астма-одно из самых распространенных хронических заболеваний дыхательной системы, значительно нарушающих качество жизни пациентов. Цель работы: проанализировать результаты научных исследований, касающихся изучения факторов риска возникновения и развития бронхиальной астмы у детей. Материал и методы. Проведен качественный анализ имеющихся в литературе, в научных базах PubMed, в реестре клинических испытаний Embase и базе библиотеки Cochrane современных данных до 2016 г. о факторах риска астмы и ее первичной профилактике. Результаты. Установлено, что на возникновение бронхиальной астмы в детском возрасте влияют не только наследственные, но и целый ряд внешнесредовых предрасполагающих факторов, которые необходимо учитывать при проведении первичной профилактики заболевания у детей из группы высокого риска. Выводы. Необходимы дальнейшие продолжительные когортные исследования на больших популяциях, которые исследуют новые и установленные факторы риска, биомаркеры фенотипов риска и БА, а также генетические и экологические взаимодействия.
