Conclusions: Elevated AFP is known to be associated with adverse pregnancy outcomes in the absence of other abnormalities. In our population this was associated with lower birthweight centile and gestational age at delivery in association with histological evidence of advanced placental maturation. Male fetuses with SK also demonstrated lower PAPP-A levels than females. Histological examination of the placenta after birth may help to corroborate the pathophysiology behind some of the adverse outcomes associated with abnormal serum biochemical markers in pregnancy. P05.08 Second trimester maternal ophthalmic artery Doppler for prediction of pre-eclampsia
Objectives: To examine the maternal renal interlobar vein Doppler performance in the first trimester of pregnancy for prediction of pre-eclampsia (PE), gestational hypertension (GH) and small for gestational age (SGA) newborn. Methods: Observational prospective cohort study with 235 singleton pregnancies between 11-13 + 6 weeks gestation. We collected maternal history data; BMI; mean arterial pressure (MAP) and Doppler impedance indices of maternal renal interlobar veins. Outcomes after delivery were divided into four groups: normal controls (n = 180), pre-eclampsia (n = 22), gestational hypertension (n = 12), newborn small for gestational age (SGA) (n = 21). Logistic regression analysis determined the contribution of tests for the early prediction of the outcomes. The detection rates (DR) to 5% and 10% of false positives rates (FPR) and the area under the ROC curve (AUC) were obtained.
Results:The DR of a model using maternal characteristics and the MAP for a FPR of 10% and 5% respectively were: All-PE 54.5% and 31.8; GH 41.7% and 33.3% and SGA 42.9% and 33.3%. The addition of renal interlobar vein Doppler did not improve the detection rate of any outcome. Conclusions: Renal interlobar vein Doppler interrogation in the first trimester of pregnancy does not appear to be a new parameter for the prediction of PE, HG and SGA.
