Resumen
Las lesiones sólidas hepáticas de nueva aparición en los pacientes oncológicos, nos obligan a descartar como primera posibilidad las metástasis hepáticas de su neoplasia primaria. Para ello, la prueba de imagen que nos lleva al diagnóstico será la resonancia magnética con contraste hepatoespecífico, que nos permitirá diferenciar entre varias entidades, entre la que se encuentra la hiperplasia nodular regenerativa múltiple asociada a oxaliplatino. Es necesario el conocimiento de los cambios hepáticos relacionados con la quimioterapia y de esta patología para así evitar procedimientos invasivos innecesarios.
Resumen
El vólvulo gástrico es una entidad rara pero potencialmente mortal, donde el estómago sufre una malrotación de todo o parte del mismo alrededor de uno de sus ejes, condicionando una obstrucción del tracto gastrointestinal superior con dilatación gástrica y riesgo de estrangulación. Las complicaciones pueden poner en peligro la vida del paciente, incluidas la necrosis y la perforación. Su diagnóstico suele ser tardío debido a sus síntomas inespecíficos, donde la evaluación de la viabilidad de la mucosa es esencial y la intervención quirúrgica urgente es obligatoria en caso de compromiso vascular.
Resumen
La infección gástrica por Helicobacter pylori (H. pylori) es muy prevalente, afectando alrededor de un 60% de la población en España. Puede producir patologías muy variadas. La lesión que más frecuentemente produce es la gastritis crónica superficial, permaneciendo asintomáticos el 90% de los pacientes infectados.
A pesar de que la infección por H. pylori es más prevalente en los pacientes con dispepsia funcional, la erradicación de la infección sólo consigue la resolución de los síntomas en un 10% de los pacientes dispépticos. Independientemente de este hecho, el diagnóstico y tratamiento de la infección por H. pylori es de primera línea en los pacientes con dispepsia funcional.
H. pylori es un carcinógeno tipo 1 según la Organización Mundial de la Salud. La infección por H. pylori se relaciona directamente con el adenocarcinoma gástrico (intestinal o difuso) y sus lesiones precursoras (gastritis atrófica y metaplasia intestinal). La erradicación de la infección disminuye el riesgo de desarrollarlos.
La infección por H. pylori también puede inducir linfoma gástrico del tejido linfoide asociado a mucosas (linforma MALT). La erradicación es el tratamiento de primera línea en estadios iniciales con buenas tasas de respuesta y supervivencia a largo plazo.
