Seven (11%) of the first 65 patients who received heart transplants at Papworth Hospital were mismatched for Toxoplasma gondii. Of these, four (57%) experienced T. gondii infection and two died. The remaining two had severe symptoms and received anti-T-gondii chemotherapy for a year after transplantation. In an attempt to reduce the impact of donor-acquired T. gondii in our heart transplant recipients, we decided in April 1984 to give prophylactic pyrimethamine to all T. gondii-mismatched patients. In this study, 7 years later, we review the efficacy of this policy. Five of 37 (14%) patients given prophylactic pyrimethamine acquired T. gondii infection; only one was symptomatic, and none died. This compares with 100% symptomatic infection in the pre-1984 patients, who did not receive prophylactic pyrimethamine. We believe that our experience has shown that pyrimethamine is effective in reducing the incidence and severity of primary donor-acquired T. gondii infection in mismatched heart and heart-lung transplant recipients.
Transcatheter closure of secundum atrial septal defect has become an alternative to surgery. We present a patient with hemodynamic collapse secondary to cardiac perforation occurring 36 h after the placement of an Amplatzer Septal Occluder and discuss complications of this kind of device.

A 62-year-old man presented with pulmonary adenocarcinoma that penetrated through the pulmonary vein into the left atrium. The tumor in the left atrium was removed via a right lower lobectomy under cardiopulmonary bypass. In selected cases, radical removal of a tumor in patients without mediastinal lymph node involvement may improve the prognosis. The use of cardiopulmonary bypass extends the possibilities of radical tumor removal.

Z 283 pacientů s Fallotovou tetralogií (TOF), kteří podstoupili radikální korekci v Dětském kardiocentru FNM v letech 1978-1988 v průměrném věku sedm let a sedm měsíců, se nám po 31-21 letech podařilo zjistit informace o 161 pacientech, což představuje dvě třetiny přežívajících. Pacientů, o kterých nemáme žádné informace, je 80 (28 % operovaných). Za dobu sledování zemřelo celkem 42 pacientů (14,8 %), z toho časná operační mortalita činila 7,4 % (21 pacientů) a k pozdnímu úmrtí došlo u dalších 21 pacientů. Dlouhodobého přežívání 26 let po radikální korekci TOF dosáhlo 85 % všech pacientů a 92 % pacientů propuštěných po korekci v dětství domů. Bylo provedeno celkem 57 reoperací u 50 pacientů. Mortalita u 26 pozdních reoperací dospělých pacientů provedených v Nemocnici Na Homolce byla nulová. Provedení reoperace bylo nutné dříve a významně častěji (v 32 %) u pacientů, u kterých byla při radikální korekci provedena incize pulmonálního prstence (s použitím transanulární záplaty nebo monokuspidální chlopně) oproti subpulmonální dacronové záplatě (p < 0,001). Vysokoškolského a středoškolského vzdělání dosáhlo 52 % pacientů ze 105. Počet plných invalidních důchodů byl významně vyšší ve skupině, která dosáhla pouze základního vzdělání (47 %), oproti skupině vysokoškoláků (0 %; p = 0,004), středoškoláků (3,4 %; p = 0,0077), ale i oproti vyučeným (10 %, p = 0,0446). Vrozená srdeční vada se vyskytla u 6,3 % dětí našich pacientů. Výskyt reziduálních, prognosticky významných nálezů je u TOF častý. Všichni pacienti, i po úspěšné korekci TOF v dětství, musejí být trvale kardiologicky sledováni. Pacienti s rizikem významných reziduálních nálezů by měli projít vyšetřením v centru pro dospělé s vrozenými srdečními vadami s otázkou nutnosti a časování reoperace. Výsledky včas a správně indikované reoperace jsou příznivé.
