El abordaje quirúrgico de procesos oncológicos que afectan al páncreas proximal suele ser complejo al tratarse de pacientes con un estado nutricional deficiente en el momento del diagnóstico que serán sometidos a una técnica quirúrgica agresiva.
Sus complicaciones postoperatorias generan un deterioro mayor con posibilidad de reintervenciones laboriosas de alto riesgo. El retraso en el vaciamiento gástrico es una de las morbilidades más frecuentes y puede obligar a investigar otras causas que prolongan la intolerancia oral.
Presentamos a un paciente que desarrolló una neumonía aspirativa secundaria a una estenosis granulomatosa sobre el asa eferente de la anastomosis gastroyeyunal en el postoperatorio de una duodenopancreatectomía cefálica complicada con una fístula pancreática. Se exponen diferentes causas y estudios diagnósticos intentando correlacionar las técnicas reconstructivas de la intervención inicial y las diferentes soluciones quirúrgicas de esta complicación.
