Een Automatische Externe Defibrillator (AED) is een relatief goedkoop hightechinstrument waarmee de medische leek een oorspronkelijk specialistische, levensreddende (nood)behandeling kan uitvoeren. Met de brede beschikbaarheid van de AED vallen dramatische verbeteringen te bereiken in de overlevingspercentages bij een cardiaal arrest. Kosteneffectiviteitsonderzoeken voor de vrij beschikbare AED zijn beperkt en ontbreken over de huisartsenpraktijk. Op theoretische gronden berekenden we dat in de huisartsenpraktijk per 1000 ingeschreven patiënten 0,05 maal per jaar een indicatie voor de inzet van een AED kan worden gesteld. Voor de huisartsenposten is dat getal 0,15. Automatic external defibrillators (AEDs) are relatively inexpensive, hi-tech devices that make it possible for non-medically trained people to perform a life-saving intervention that was originally performed only by medical specialists. The broad availability of AEDs can dramatically increase the proportion of people who survive cardiac arrest. Few cost-effectiveness studies of the available AEDs have been performed and none in a general practice setting. We have estimated that use of an AED is indicated 0.05 times a year per 1000 patients registered in a general practice, and 0.15 times for out-of-hours general practice services. The decision whether a general practice or out-of-hours service should invest in such a device is probably influenced more by social factors than by cost-effectiveness.
SamenvattingReanimatie tijdens de dagpraktijk of tijdens diensten op de huisartsenpost blijft voor de huisarts en zeldzame maar belastende gebeurtenis. Beschreven wordt het belang van een goede organisatie, oefening en scholing van alle aspecten die bij een reanimatie aan bod komen. Tot slot wordt ingegaan op het stoppen of het niet starten van een reanimatie. InleidingBij volledige uitval van de vitale functies van een patiënt mag men met recht van een calamiteit spreken. Alleen reanimatie kan de patiënt doen overleven. Het uitvoeren van reanimatiehandelingen is relatief eenvoudig. Juist omdat reanimaties op een huisartsenpost weinig voorkomen en een huisarts relatief zelden betrokken is bij het uitvoeren van reanimatiehandelingen, is het nodig deze handelingen regelmatig te oefenen.Het is binnen de beroepsgroep gebruikelijk dat huisartsen en doktersassistentes regelmatig getraind worden in reanimatiehandelingen. Maar een reanimatie houdt meer in dan het uitvoeren van reanimatiehandelingen.Juist die andere, organisatorische aspecten zorgen bij beroepsbeoefenaars vaak voor een negatieve beleving van reanimatie. Oplossingen voor dit probleem zijn te vinden door het probleem te analyseren. Protocollen voor calamiteiten op de huisartsenpostHet is noodzakelijk om op de huisartsenpost een vaste taakomschrijving te hebben voor calamiteiten (zoals een reanimatie), waarin vastligt welke taken in zo'n geval zijn toebedeeld aan elke functie. Doel Vergelijken van medisch handelen tussen huisartsen met en zonder tropenwerkervaring.Methode Alle huisartsen in de DHV's Utrecht en Noordwest-Veluwe (n=517) werd gevraagd of zij ten minste twee jaar als arts in de tropen hadden gewerkt. Per praktijk werden vier indicatoren van huisartsgeneeskundig handelen verzameld via declaratiebestanden. Deze indicatoren werden vergeleken tussen de groep met en zonder tropenervaring. De uitkomsten werden gecontroleerd voor een aantal sociodemografische factoren.Resultaten Bijna 8% van de onderzoeksgroep had tropenervaring. Het gemiddeld aantal verwijzingen door deze huisartsen was 9,5% lager. De gevonden verschillen in het aantal diagnostische verrichtingen (-12%) en voorschriften (-7,5%) bleken niet significant. Voormalige tropenartsen deden meer verrichtingen in de eigen praktijk. Bij multivariate analyse bleek tropenervaring in tegenstelling tot een aantal sociodemografische factoren geen significante invloed te hebben.Conclusie Tropenervaring heeft in kwantitatieve zin geen invloed op het verwijs-en voorschrijfbeleid, noch op het aanvragen van aanvullende diagnostiek door de huisarts.Tot voor kort vertrokken jaarlijks 50-60 Nederlandse artsen naar ontwikkelingslanden om daar als tropenarts te gaan werken. Deze artsen zijn over het algemeen goed voorbereid doordat zij veelal een door de Nederlandse Vereniging van Tropische Geneeskunde gesuperviseerde tropenopleiding hebben gevolgd. Deze opleiding bestaat uit twee jaar klinische stages in verloskunde en chirurgie en de nationale tropencursus. Na een eerste contract van ten minste 2 jaar keert circa 90% van de tropenartsen terug voor een definitieve loopbaan in de Nederlandse gezondheidszorg. Ongeveer een derde van hen wordt huisarts. 1Veel artsen doen werkervaring op in de tropen doordat zij voor korte tijd worden uitgezonden in het kader van co-schappen, wetenschappelijke stages, of noodhulpprojecten. Naar schatting heeft 5-10% van de Nederlandse artsen een periode in een ontwikkelingsland gewerkt. 2Volgens veel tropenartsen is werken als arts in een derdewereldland een verrijking van de professionele en persoonlijke ontwikkeling.3-6 Er wordt een stevig beroep gedaan op handelingsbekwaamheid, creativiteit en zelfredzaamheid van de tropenarts aangezien deze met weinig middelen onder minimale condities medische zorg moet verlenen. Door vertrouwdheid met niet-inheemse ziekte, openheid voor participatie en kostenbewustzijn zou de ex-tropenarts na repatrie¨ring een specifieke en zinvolle bijdrage kunnen leveren aan het Nederlandse zorgsysteem. Niet iedereen is echter overtuigd van deze meerwaarde.7 De discussie over de formele erkenning van de tropenopleiding en het verlenen van vrijstellingen voor de opleiding tot huisarts is hier een indicatie van. 8-12Wat is bekend? 1.
