Rara vez es necesario tratar la hepatitis B de manera inmediata. Sin embargo, hay algunas situaciones cl nicas que requieren una terapia inmediata o incluso urgente para impedir la progresi on de la enfermedad, reducir la morbilidad o estabilizar cl nicamente al paciente. En este trabajo se revisan estas indicaciones, comenzando por las areas en las que la evidencia de beneficio terap eutico es mayor. El lector puede consultar las indicaciones menos urgentes en las actuales gu as de pr actica cl nica de la Asociaci on Americana para el Estudio de las Enfermedades Hep aticas (AASLD)1 . Fibrosis avanzada y cirrosis compensadaEn todos los pacientes con enfermedad activa y fibrosis avanzada (estadio Ishak 4) est a indicado el inicio inmediato de la terapia con un an alogo de nucle osidos (Tabla 1). De los pacientes incluidos en los estudios de registro para los an alogos de nucle osidos actualmente aprobados, entre el 10% y el 15% presentaban cirrosis en la biopsia basal. En los estudios fundamentales de tenofovir, un 20% adicional presentaba enfermedad en estadio Ishak 4 2 . En todos estos estudios se observ o mejor a de la fibrosis despu es de un tratamiento relativamente breve (48-52 semanas). Recientemente se ha realizado un amplio an alisis sobre el grado de mejor a histol ogica asociado a la administraci on de tenofovir o entecavir a largo plazo. Se dispon a de biopsias pareadas de 328 de los pacientes (51%) que recibieron tenofovir durante 4 a 5 años 1 y se observ o mejor a histol ogica (2 puntos de mejor a en la puntuaci on necroinflamatoria de Knodell sin aumento de la fibrosis) en el 95% de los 235 pacientes no cirr oticos. Tambi en se dispon a de las biopsias hep aticas de 93 pacientes con cirrosis basal (estadio Ishak 5 o 6), el 72% de los cuales presentaron una reducci on 2 puntos en el estadio de fibrosis en la semana 240 de tratamiento. Aunque los datos no son tan impactantes, tambi en se registr o reversi on de la cirrosis despu es de 3 años de tratamiento con entecavir 3 .Se ha demostrado que la situaci on cl nica de los pacientes con fibrosis avanzada mejora con la supresi on viral continua. En un ensayo cl nico multinacional, controlado con placebo, realizado en pacientes asi aticos portadores del ant geno de superficie de la hepatitis B (HBsAg) con un ndice Ishak de fibrosis 4 (que inclu a a 400 pacientes con cirrosis), el n umero de pacientes que presentaron un aumento 2 puntos en la escala de Child-Turcotte-Pugh (CTP) y diagn ostico de carcinoma hepatocelular de novo fue significativamente inferior entre los pacientes tratados con lamivudina tras una mediana de 32 meses 4 .Considerados en su conjunto, estos resultados son enormemente alentadores. A un queda por determinar si la reversi on de las caracter sticas histol ogicas de la cirrosis contribuye de forma independiente a disminuir el riesgo de carcinoma hepatocelular o si reduce las complicaciones cl nicas asociadas a la hipertensi on portal establecida. The potential immunoregulatory effects of rosette inhibitory factor (RIF) were evaluated using lymphocyte subpopulations defined by monoclonal antibodies (OKT4, OKT8). Initial experiments indicated that RIF inhibits E-rosette formation by T cells that provide helper/inducer function (OKT4+). Subsequently, experiments were done to assess if RIF had an inhibitory effect on in vitro immunoglobulin and anti-HBs production. These studies used peripheral blood mononuclear cells from convalescent and hepatitis B-vaccinated donors. The results indicated that RIF-positive low-density lipoprotein, but not RIF-negative low-density lipoprotein, inhibits in vitro production of IgG and IgM by 60 to 70% and totally ablates anti-HBs synthesis. In order to determine if RIF directly affected the proliferative response of T4 lymphocytes, these cells were cultured in the presence of optimal mitogen concentrations and either RIF-positive or RIF-negative low-density lipoprotein. The experiments demonstrated that RIF-treated T4 cells are hyporesponsive to pokeweed mitogen and phytohemagglutinin when compared to T4 cells cultured in the absence of RIF. In contrast, B cell proliferation in response to soluble T helper factors was not affected by incubation with RIF. As the production of soluble helper factors by T4 cells was inhibited by exposure to RIF-positive but not RIF-negative low-density lipoprotein, we conclude that the suppressive effect of RIF on immunoglobulin and anti-HBs synthesis is due to inhibition of this specific helper T cell function.
